Lozap. I have never suffered from problems with blood pressure. I joked that as an astronaut it’s always 120 to 80. And it was like that until I was 40 years old. But six months ago I began to experience a strong deterioration in my health due to the weather. If a cataclysm was expected in nature, headaches and heaviness in the back of the head began. It got so bad it was almost nauseating.

Overdose

Symptoms of a drug overdose include the following:

  • dehydration and loss of electrolytes.

Suction

Losartan is well absorbed when taken orally, and during the “first pass” through the liver it is metabolized by carboxylation with the participation of the cytochrome CYP2C9 isoenzyme with the formation of an active metabolite.

Bioavailability of losartan (systemic) is about 33%. In the blood serum, Cmax of losartan and its active metabolite are detected after an hour and 3-4 hours after oral administration, respectively. The bioavailability of losartan is not affected by food intake.

Distribution

Binding to plasma proteins is >99%, predominantly to albumin. Vd - 34 l. Losartan almost does not penetrate the BBB.

Metabolism

Approximately 14% of losartan taken orally or intravenously is converted into an active metabolite.

Removal

The clearance of losartan (plasma) is 600 ml/min, and 50 ml/min for the active metabolite.

Renal clearance of the active ingredient and its active metabolite is 74 ml/min and 26 ml/min, respectively. About 4% of the oral dose is excreted unchanged through the kidneys and about 6% through the kidneys in the form of an active metabolite.

Losartan and its active metabolite are characterized by linear pharmacokinetics when administered orally in doses up to 200 mg. Concentrations of losartan (plasma) and its active metabolite from the moment of oral administration decrease polyexponentially, with the final half-life of losartan being 2 hours and its immediate active metabolite being 6-9 hours.

In plasma, neither losartan nor the active metabolite practically accumulates at a dose of 100 mg/day Lozapa. Losartan and its metabolites are eliminated from the body through the intestines and kidneys.

Pharmacokinetics in certain cases

The concentration of losartan in moderate and mild alcoholic cirrhosis of the liver was 5 times higher, and the active metabolite was 1.7 times higher, compared with healthy male volunteers. In blood plasma, the concentration of losartan with CC >10 ml/min is identical to that with normal renal function. The AUC is twice as high in individuals requiring hemodialysis than in individuals with normal renal function.

Neither losartan nor carboxylic acid (E-3174), which is its main active metabolite, is removed from the body during hemodialysis.

In the blood plasma, the concentrations of losartan and its immediate active metabolite in men with hypertension, both elderly and young, do not differ significantly.

Plasma concentrations of losartan in women with hypertension are twice as high as in men with hypertension.

What to replace if Lozap does not hold pressure?

How to replace Lozap if it does not hold pressure is a common question when the product ceases to perform its functions. Specialists replace it with other medications with the same indications according to the instructions for use.

  • Losartan potassium (50 mg). This is an angiotensin II hormone receptor blocker (AT1 subtype). It does not inhibit kinase II, an enzyme that breaks down bradykinin. The blocker reduces peripheral vascular resistance, afterload, aldosterone and adrenaline levels. Prevents the development of myocardial hypertrophy. Helps patients with heart failure increase exercise tolerance. Losartan is absorbed into the blood from the intestinal tract and is broken down in the liver, releasing an active metabolite. It reacts with blood plasma protein. The maximum concentration of the substance in the blood is observed 60 minutes after administration. Metabolism occurs within 3-4 hours. The component is excreted after 2 hours: with urine – 34%, and from the intestines – 66%.
  • Hydrochlorothiazide (12.5 mg). This is a thiazide diuretic that reduces sodium absorption. Helps remove potassium, phosphate and bicarbonate from the body unchanged in the urine for 6-14 hours after taking the drug. Helps lower blood pressure because it changes the reactivity of vessel walls, reduces the volume of circulating blood and increases depressor pressure. The component dissolves in the stomach and enters the blood, slowing down the removal of potassium ions from the body. Almost 61% of the substance is excreted unchanged by the kidneys.

The excipients are:

  • microcrystalline cellulose (MCC) (210 mg);
  • mannitol (89 mg);
  • povidone (7 mg);
  • magnesium stearate (3.5 mg);
  • croscarmellose sodium (18 mg).
