special instructions
With long-term use of the drug, as with long-term use of other antimicrobial agents, the development of superinfections caused by drug-resistant microorganisms, including fungi, is possible.
Long-term instillation of the drug into the eyes can lead to thinning of the cornea with the development of its perforation, as well as to an increase in IOP. Treatment with drugs containing corticosteroids should not be repeated or prolonged without regular monitoring of IOP and eye examination for the development of cataracts or secondary infections.
Local corticosteroids should never be used in patients with ocular hyperemia of unknown etiology, because inappropriate use of the drug can lead to significant deterioration of vision (see “Contraindications”).
Framycetin sulfate, which is part of the drug, is an antibiotic from the group of aminoglycosides, which are characterized by the development of nephro- and ototoxic effects when used systemically or topically on open wounds or damaged skin. These effects are dose dependent and are increased in renal or hepatic impairment
Although the development of these effects was not observed when the drug was instilled into the eyes, the possibility of their occurrence should be taken into account in the case of topical use of high doses of the drug in children
The duration of use of the drug should not exceed 7 days, except in cases of obvious positive dynamics of the disease, because long-term use of GCS, which is part of it, can mask hidden infections, and long-term use of antimicrobial components can contribute to the emergence of resistant microflora.
The bottle must be closed after each use. Do not touch the tip of the pipette to your eye. After opening the bottle, the drug should be used within 1 month.
Patients who temporarily lose clarity of vision after instilling the drug into the eye are not recommended to drive a car or work with complex machinery, machines or any other complex equipment that requires clear vision immediately after instilling the drug.
Drug Sofradex
This is a combination type remedy used for eye and ear diseases as part of complex therapy or independently. This drug is prescribed to both adults and children. It is only suitable for treating existing problems and is not used as a preventive measure. You can purchase the product at any pharmacy without a prescription.
Compound
The drug is available only in the form of drops for topical use. Sofradex is sold in dark glass bottles with a capacity of 5 ml. The kit additionally includes a convenient dropper, which is placed on the bottle after the first opening, and instructions with a full description of the product.
Sofradex ear drops contain three active ingredients:
- gramicidin;
- dexamethasone;
- Framycitin sulfate.
The composition also includes additional components: lithium chloride, polysorbate 80, citric acid monohydrate, sodium citrate, ethanol and phenylethanol, as well as specially prepared water.
How it works
Sofradex effectively fights staphylococci
The drug stops the development and activity of bacteria and has an antimicrobial and antistaphylococcal effect. The product helps relieve swelling and hyperemia, eliminate inflammation, improve the outflow of discharge from the ear canal, and has an antihistamine effect. This drug relieves almost all symptoms of inflammatory processes affecting the eyes and ears.
Gramicidin is an antibiotic that, when applied topically, effectively fights pathogens and enhances the effect of other components of the product. Framycitin in the form of sulfate is an even more potent component that has an antimicrobial and bactericidal effect, but differs in that the resistance of microorganisms to it takes a long time to develop. And, therefore, therapy with it in its composition brings in most cases a quick and long-lasting result.
Dexamethasone is a glucocorticosteroid drug, which, when applied topically, relieves swelling and inflammation, eliminates pain and other disturbing symptoms, and accelerates tissue healing. In addition, the complex effect of these three active ingredients helps stop lacrimation.
Important! Sofradex is not instilled into the ears and eyes if the disease is of a viral nature or fungal etiology.
Can Sofradex be dripped into the nose of children?
Sofradex nasal drops are prescribed with caution to children, but in some cases it is difficult to do without them. The only exceptions are infants, for whom the drug is contraindicated.
This is due to the fact that Sofradex is a hormonal drug and therefore has many side effects. Long-term use of the drug is fraught with dysfunction of the adrenal cortex.
Despite the fact that Sofradex drops are considered eye and ear drops, they have long established themselves as an effective remedy for a runny nose. That is why doctors in many cases prescribe them for children to instill into the nose. But still, the use of Sofradex for a runny nose, which is not complicated by other pathologies, is extremely undesirable. Instillation of the drug into the nose of children is advisable only in the presence of the following pathologies:
- nasopharyngitis,
- runny nose of bacterial origin, which is difficult to treat,
- inflammation of the adenoids,
- allergic rhinitis.
If children have a simple runny nose, it is better to use safer remedies that have minimal side effects.
Recommendations for use
Sofradex is a product that can only be used as prescribed by a doctor. In adults, chronic herpetic infection often worsens, for which this drug is contraindicated.
The duration of the course of treatment is a week, since after this the bacterial flora may develop resistance to the drug. Outwardly, this may not manifest itself in any way, since dexamethasone eliminates inflammation, pain and itching, but at the same time reduces immunity and the infection can spread to the internal structures of the hearing organ. With prolonged use, there is also a risk of activating a fungal infection.
Who should not use it
There are times when you cannot use Sofradex (eye drops). The instructions have a list of contraindications. The doctor does not prescribe this remedy for viral and fungal diseases, including herpetic lesions of the cornea with the formation of ulcerations, tuberculosis, purulent inflammatory process in the eyes, the development of glaucoma with increased intraocular pressure.
Eye drops are contraindicated in cases of increased susceptibility to the active and auxiliary components of this solution, in case of insufficient functioning of the kidneys and liver, in case of chronic chlamydial infection with damage to the conjunctiva and ocular cornea, in case of changes in the integrity of epithelial cells in the cornea and a decrease in the thickness of the outer membrane covering the eyeball.
Treatment of eye diseases with Sofradex in young children is carried out with extreme caution, and this drug is not prescribed to infants, pregnant and breastfeeding women due to unproven safety. Long-term use of drops for topical treatment of eye diseases can have a general systemic effect on the body
Long-term use of drops for topical treatment of eye diseases can have a general systemic effect on the body.
The work of patients associated with driving vehicles or other mechanisms, requiring increased attention from the visual organs, cannot be combined with treatment with Sofradex (eye drops). The instructions describe the manifestation of blurred vision or other deterioration in eye function.
Contraindications for use
As with any other antibiotic, Sofradex drops have certain contraindications for use. They are prohibited for use if there is a history of the following pathological conditions:
- hypersensitivity to the active substances, as well as to the auxiliary components of the drug;
- violation of the integrity of the eardrum, since when active components get into the wounds, hearing loss may occur;
- the presence of concomitant viral and fungal infections, especially tuberculosis and trachoma;
- damage to the outer stratum corneum and sclera of the orbit;
- herpes simplex keratitis - against the background of the development of pathology, an increase in the diameter of the eye ulcer is observed, which can provoke partial or complete loss of vision;
- glaucoma.
The drug is also contraindicated for use by pregnant women and patients during lactation. In addition, Sofradex is not allowed for use in children under 2 years of age. If there is an urgent need to take an antibiotic, the doctor should regularly monitor the child’s condition and his reaction to taking the drug throughout the entire therapeutic course.
For reference! Prolonged and uncontrolled use of drops can lead to the development of systemic side effects, in particular from the adrenal glands. Therefore, treatment of children with the antibiotic Sofradex is carried out under the supervision of a doctor.
Analogs for therapeutic effects for the eyes
Substitutes for Sofradex are those medications that have the same therapeutic effect, but a different composition. Analogues of Sofradex eye drops are the medications Tobradex and Maxitrol, which are effectively used in ophthalmology.
Tobradex
Tobradex is produced in drops and ointments based on tobramycin and dexamethasone. The drug is used for the prevention and treatment of the following ophthalmological disorders:
- eye pathologies of bacterial etiology – blepharitis, keratitis;
- conjunctivitis of bacterial and allergic origin.
Blepharitis
Tobradex is also prescribed in the postoperative period after ophthalmological operations, after injury to the eye, to prevent the development of pathologies or infection.
The drug should not be prescribed if you are allergic to its components, or if the following factors are present:
- fungal and tuberculosis pathologies;
- herpes;
- children's age - less than 1 year.
Prescribed with caution in the treatment of patients with glaucoma or depletion of the cornea. Contraindicated for use in women during pregnancy and breastfeeding due to the unclear safety of the drug, the level of influence on fetal formation and the concentration of active components in breast milk.
Tobradex is used to instill the medicine into the conjunctival sac, 1-2 drops 5-6 times a day. At the first symptoms of the acute phase of the disease, it is necessary to drip 2 drops every 2-3 hours.
This treatment regimen can last no more than 2 days. The ointment must be applied to the conjunctival sac 3-4 times a day. To increase effectiveness, drops + ointment should be used in treatment.
Maxitrol
Maxitrol is a combined medication with anti-inflammatory and antibacterial effects. The main components are dexamethasone, neomycin sulfate and polymyxin.
Polymyxin B is an antibacterial substance that binds to phospholipids of affected cell membranes and destroys them. Dexamethasone is a glucocorticoid medication that has anti-allergic and anti-inflammatory properties. The component has a desensitizing effect on the body.
Pharmacokinetics and indications for use
Maxitrol is prescribed for infectious and inflammatory eye pathologies:
- inflammatory conjunctivitis;
- conjunctivitis of bulbar etiology;
- inflammation in the eyeball, its anterior segment;
- infection or inflammation of the cornea, which caused the organ to be exposed to radiation waves, chemical and thermal burns;
- eye injury from foreign objects.
Maxitrol should not be used in the treatment of:
- chickenpox;
- viral eye pathologies;
- herpes;
- patient allergies to medication;
- bacterial infection;
- fungal infections;
- pregnancy;
- lactation.
Chickenpox
Maxitrol is prescribed locally, 1-2 drops 5-6 rubles/day (drop into the conjunctival bursa of the eye). In case of severe pathology, the frequency of administration can be doubled - the instillation procedure can be carried out every 2 hours.
The negative effects of Maxitrol and Tobradex eye drops appear infrequently and are expressed in burning and itching of the eyes, as well as the development of eye pathologies:
- increased pressure inside the eye;
- thinning of the cornea;
- development of cataracts.
How to use correctly
To avoid complications, nasal drops can only be used as prescribed by a doctor, who will select the required dosage. Otolaryngologists usually recommend diluting the drops with saline solution (1: 1), and then instilling the mixture 4 drops 3 times a day for no more than seven days. The doctor may prescribe longer-term use, as well as individually selected treatment regimens that should not be repeated on your own.
For the treatment of chronic diseases (adenoiditis, sinusitis), the use of Sofradex is sometimes combined with dexpanthenol (1% Moreal Plus nasal spray). This is a medicine with a restorative and mild anti-inflammatory effect. But at least an hour should pass between the administration of drugs: the simultaneous administration of dexpanthenol and antibiotics increases the risk of developing allergies. This drug is not contraindicated for children.
Price of Sofradex and its analogues
It must be said that the issue of price when choosing Sofradex or its analogue, including a cheaper one, is definitely not the key one.
Thus, when choosing a specific drug, factors such as individual tolerability of the drug and the presence or absence of contraindications become key. This is what should determine the patient’s choice of which particular drug, “Sofradex” or its analogue, he should give preference for use for instillation in the ear, eye or nose. The issue of price is rather of secondary importance, since the price for Sofradex is also low. The main determining factor should be the instructions received from the doctor.
It would also be useful to focus on your own feelings. So, if instilling the drug into the ear, eye or nose is accompanied by severe pain (when using Sofradex as ear drops, this is possible, in particular, with purulent otitis media, for which it is contraindicated), you should immediately stop further use of the medicine and, possibly, In some cases, replace it with an analogue with the same effect. Similar situations may arise when treating eye diseases or diseases of the nasopharyngeal mucosa with Sofradex.
Among the disadvantages of Sofradex, which make one think about finding its analogues, many patients cite not the cost of the drug, but its features, such as, for example, an unpleasant odor. However, the key importance, naturally, should be the effectiveness of the drug (patients are practically unanimous on this - in the vast majority of cases the drug acted quickly and reliably) and the recommendations of the attending physician, whose instructions should be the main factor influencing the choice.
Sofradex price

Directions for use and dosage
The dosage depends on the type of dosage form and the purpose of administration.
To treat eye inflammation, inject 1-2 drops into the conjunctival sac with an interval of 4 hours between instillations. This dosage is used for mild cases. For pronounced symptoms, 1-2 drops are instilled every hour. With the disappearance of symptoms, the dosage is reduced.
Otitis is treated by instilling 2-3 drops into each ear three times. After insertion, the ear canal is closed with sterile gauze or cotton wool. If discomfort occurs during the procedure, it is recommended to use turundas made of sterile materials. Turundas are replaced with new ones soaked in solution.
For rhinitis, drops are used in a special course - up to 5-7 days, 1-2 drops are instilled into each nostril.
For inflammation of the adenoids, the dosage is usually increased to 4 drops in each nostril. For adenoiditis, the course of treatment is extended to 10 days.
Conditions of use in the treatment of adenoids involve the use of a nebulizer. In this case, there are several nuances.
The Sofradex solution is prepared with distilled water in a ratio of 1 to 3. The nebulizer is switched to the nasal nozzle mode. When using a nebulizer, solution vapors pass through the oral cavity to the adenoids in small quantities. Exposure through the nose provides complete access to the adenoids.
The nebulizer is used with caution in small children. The main purpose of inhalation treatment is to deliver the drug directly to areas of inflammation
In this case, it is necessary to observe safety precautions.
The medicinal solution is prepared before use. If liquid drips from any part of the device, stop using it.
Features of reception in children
Manufacturers warn that the use of the medicine for newborns is possible only if there are no contraindications for this. A spray is prescribed for children. This method allows you to freely instill the product into the baby’s nose. Inhalation therapy with Sofradex is a common method for the treatment of rhinitis, otitis and adenoiditis in young children.
Can pregnant women take it and during breastfeeding?
Do not use during pregnancy if the risk of complications for fetal development outweighs the benefits of using the drug.
The period of breastfeeding is also a contraindication to taking the drug.
Are there possible side effects?
Usually, correct use of the drug Sofradex as prescribed by a specialist does not lead to the development of side effects. In rare cases, the following negative reactions are possible:
- allergic reactions of a local and general nature, including itching, burning, swelling and redness;
- impaired sensitivity, including dullness of hearing and blurred vision (usually goes away without any additional measures 10-15 minutes after the instillation procedure).
With prolonged and uncontrolled use of Sofradex, a decrease in visual and hearing acuity, damage to the optic nerve, and the development of glaucoma are possible. In addition, there may be a decrease in local immunity and disruption of the local microflora, which can cause the development of bacterial infections and fungal infections of the ears and eyes. Also in this case, there is a risk of developing systemic effects and disruption of the adrenal glands.
Important! If any of these effects are detected, except for a temporary decrease in sensitivity, you must stop using the drug and immediately consult a doctor. Most likely, the specialist will select another drug with a similar effect for treatment.
Side effects from Sofradex eye drops
As patient reviews show, treatment is usually not accompanied by unwanted reactions. On the contrary, unpleasant symptoms disappear quickly, the inflammatory process is effectively eliminated, and along with it, itching, redness and burning in the eyes disappear. However, sometimes side reactions still occur, which are expressed as follows:
- tingling in the eyes;
- slight itching immediately after instillation;
- decreased visual acuity (usually a temporary phenomenon, passes quickly);
- a veil before the eyes and blurred vision.
Doctors warn that in individual cases systemic negative manifestations may occur. These include the following symptoms:
- damage to the optic nerve;
- if the microflora of the eye is disturbed, which is possible with uncontrolled use of the drug, then there is a risk of fungal disease;
- thinning of the membrane of the eye.
As patient reviews show, mildly disturbing undesirable symptoms usually go away on their own within 15-20 minutes. But experts advise not to drive immediately after using the drops and to refrain from working with mechanisms that require extreme concentration. You need to wait about an hour and make sure there are no sudden reactions. If severe complications occur or are suspected, the drug should be discontinued and consult an ophthalmologist.
List of contraindications and side effects from the use of Sofradex drops
The drug Sofradex has a number of contraindications:
- diseases accompanied by purulent processes,
- viral and fungal infections,
- tuberculosis,
- perforation of the eardrum,
- glaucoma, if drops are used to treat eye diseases,
- herpetic keratitis,
- the patient's age is less than three years,
- intolerance to drug components,
- the presence of damage to the stratum corneum of the eye or sclera,
- pregnancy and lactation period.
The main side effects of the drug
Patients rarely experience any side effects when using ear drops. Their appearance is usually associated with non-compliance with the instructions for the drug or with the presence of individual intolerance to its components. The most common side effects after using Sofradex are:
- the appearance of discomfort and burning sensations,
- development of itching and severe pain,
- dullness of hearing if the product is instilled into the ear canal,
- blurred vision when using drops to treat eye diseases.
These side effects from the use of the drug do not pose a danger to human health. They usually disappear on their own within 15 minutes after using the drops.
Side effects of Sofradex with long-term use
If the patient neglects the instructions and recommendations of doctors, carrying out treatment for longer than a week, severe systemic side effects may develop:
- development of glaucoma,
- loss of quality of vision,
- damage to the optic nerve, which leads to serious vision problems,
- disruption of normal microflora, which leads to the development of fungal infection,
- the addition of an additional secondary infection that is resistant to the antibiotics used,
- decreased hearing quality.
Long-term use of Sofradex causes dysfunction of the adrenal glands. Therefore, if any alarming symptoms appear, you should stop taking the drops and consult a doctor.
pharmachologic effect
The drug Sofradex is a combination drug and has the following therapeutic effects:
- Antibacterial action.
- Anti-inflammatory effect.
- Antiallergic effect.
Sofradex drops and ointment contain two antibiotics - framycetin and gramicidin, from the group of aminoglycosides. These antibiotics effectively destroy a wide range of pathogenic microorganisms that cause infectious and inflammatory diseases of the ears and eyes. Framycetin destroys a large number of gram-positive bacteria, including Staphylococcus aureus, which is an opportunistic microbe that often causes inflammatory lesions in various organs and is quite difficult to cure. Framycetin also has a detrimental effect on the main gram-negative microorganisms - Escherichia coli, dysentery, Proteus, etc. Resistance to this antibiotic develops very slowly. Framycetin does not destroy streptococci, fungi and microbes that can live in an oxygen-free environment (anaerobic). The second antibiotic of Sofradex, gramicidin, increases the spectrum of action of framycetin, acting especially well against staphylococci, which are difficult to treat.
The third component of Sofradex ointment and drops is the glucocorticoid hormone dexamethasone, which has the ability to suppress the inflammatory response, reduce the production of substances that stimulate inflammation, and reduce the degree of permeability of the capillary wall. In addition, dexamethasone has the property of suppressing allergic reactions and itching. Dexamethasone also relieves swelling, improving nasal breathing, hearing and vision.
Sofradex, when injected into the eyes, relieves pain, eliminates burning, lacrimation and photophobia. And the introduction of the drug into the ears eliminates skin redness, soreness, itching, burning in the external auditory canal and the feeling of a stuffy ear.
What does the drug consist of and how does it work?
Saphrodex eye and ear drops (Aventis Pharma, India) have a combined composition: two antibiotics and a glucocorticoid hormone (GCS).
The antibiotics framycetin and gramicidin act synchronously, killing and suppressing the vital activity of most gram-positive and gram-negative bacteria that cause infections of the nasopharynx, hearing and vision. At the same time, framycetin can have a toxic effect on the internal structures of hearing (ototoxic effect), so the medicine is allowed to be used only if the eardrum is intact, which must be confirmed by an ENT doctor. Antibiotics do not work on viruses and fungi.
Dexamethasone is a corticosteroid with a pronounced anti-inflammatory and desensitizing effect. It is able to instantly relieve inflammatory and allergic tissue swelling, as well as pain and itching associated with swelling. But all GCS suppress local tissue immunity and therefore require careful use. If the drug is used for viral infections, a protracted course of the disease is possible. With prolonged use of these drops, the fungal infection that is always present on the mucous membranes of the ENT organs begins to actively multiply.
How to use Sofradex eye drops
The usual course of therapy with Sofradex does not exceed a week (2 drops in each eyeball). If the therapeutic doses prescribed by the ophthalmologist are observed, side effects rarely develop, and the medicine is well tolerated by patients. Depending on where the instillations will be made, the doctor individually selects the dose of the drug.
In the ears
The use of Sofradex in the treatment of pathological processes localized in the ear area occurs according to the following scheme:
- Instillations are performed immediately, in two ears;
- During the day, you need to repeat this procedure 4 times (3 drops in each ear);
- To relieve pain and increased irritation of the ear canal, after the manipulation, a cotton or gauze turunda is inserted into its lumen (it can also be moistened with Sofradex solution). The time it spends in the auricle should not exceed 15 - 20 minutes.
Sofradex drops give positive dynamics for:
- Inflammatory process of the external ear (otitis), in any form.
- Bacterial infection.
If, after performing ear drops, a feeling of unbearable pain occurs, this may indicate a violation of the integrity of the eardrum. In this case, the use of the medicine is stopped, and it becomes necessary to consult an ophthalmologist again. Each subsequent instillation procedure must be accompanied by the introduction of a new, sterile turunda.
Nasal passages
This method of administration of this dosage form occurs in the treatment of adenoids.
It is very important, when establishing such a diagnosis, not to allow self-treatment, since there is a form of adenoids that is not amenable to medication
In order for the medicine to have a positive effect, the following rules must be followed:
- Before the procedure, be sure to rinse the nasal passages with sodium chloride solution; if the patient is too small, they are cleaned with a syringe. Older children are advised to blow their nose.
- The procedure is always performed in a lying position. This is so that the maximum volume of drops reaches its goals and does not flow out of the nose.
- After performing the manipulation, the patient is recommended to lie quietly and not roll over on his side.
Basically, the following standard scheme is used:
- The first 10 days, after breakfast, lunch and dinner, eye drops are instilled (4 in each nasal passage);
- The next five days, the manipulation is repeated twice a day, the dose is reduced to 2 drops;
- From day 16 to day 20, Sofradex is applied once in the amount of 2 drops.
There is a second method of treating adenoids. It involves nasal inhalation of the medication using a nebulizer. A steam evaporator is not used in this case, since high temperature destroys the chemical composition of the antibacterial components of Sofradex drops.
In the corner of the eye closer to the bridge of the nose
This method of instillation can be used for the following diseases:
- Bacterial conjunctivitis. If the patient develops this form of the pathological process, accompanied by an acute onset, on the first day of treatment it is possible to use drops every hour. They are instilled into the conjunctival sac, 2 or 3 drops each. As the severity of the process decreases, the time interval between eye drops increases;
- Conjunctivitis of allergic etiology. To eliminate the clinical picture of this disease, Sofradex is instilled into the eyeballs every 4 hours, no more than 2 drops into each corner of the eyes;
- Iridocyclitis. In the first days of therapy, the frequency of instillation should not exceed 2 hours. Two drops are dropped into one eye. After removing the symptoms of the acute process, a standard dosage is prescribed (1 - 2 drops, 3 or 4 times a day).
In severe cases of the disease, eye drops can be used for up to 2 weeks. In some cases, after instilling Sofradex into the eyes, an unpleasant sensation resembling a burning sensation may appear. Any other negative symptoms require urgent consultation with a doctor. Since it may indicate intolerance to this type of eye drops.
Reviews
- Inna, 32 years old: “My otitis media is chronic. The fact is that I did not manage to get to the hospital on time, since I decided that I could cope with the pain and other symptoms myself. But I was unable to get the desired effect. The pain began to intensify, ear congestion arose, and purulent discharge appeared. When I went to the doctor, he diagnosed me with chronic otitis media. Unfortunately, I will no longer be able to completely get rid of the disease, but I can alleviate the course of the exacerbation stage and prolong remission with the help of Sofradex. I don't drip them, but put a gauze pad in my ear. I carry out this manipulation 2 times a day.”
- Maria, 27 years old: “I always have Sofradex in my medicine cabinet. And the reason for this is my chronic otitis media. When I reach the stage of exacerbation, my usual rhythm of life is disrupted, because due to severe pain and discomfort I cannot fully work or sleep. Sofradex drops are what save me. I put them in my ear 3 times a day. I can notice a positive effect the very next day. The pain decreases, the amount of discharge also decreases. Of course, you can’t use it for a long time, otherwise the bacteria will get used to it and you won’t get the desired effect. My dad also uses these drops to relieve inflammation and irritation, because he works for me as a welder, and his eyes regularly get irritated.”
- Valentina, 54 years old: “With the help of Sofradex drops, I managed to cure otitis media and get rid of unpleasant symptoms. I didn't go to the doctor. My godfather is a therapist, so she recommended Sofradex to me. In the morning and afternoon, I dripped 5 drops into each ear canal, but before going to bed, I soaked a gauze swab in the solution and inserted it into the ear. To ensure that the effect of the drops used is quick and high-quality, you should thoroughly clean the ear canals before using the drug. In addition, I began to notice that when using Sofradex I was able to prolong remission and live a full life.”
Sofradex is an effective ear drop for the treatment of otitis media. They can be used by children and adults. Cases of overdose are rare. As a rule, they occur due to an increase in the dosage of the drug indicated by the doctor. The uniqueness of the medicine is that the patient can feel relief the very next day. If side effects occur, you should immediately inform your doctor and stop using the drops.
Medicine Aprolat

Sofradex instructions for use
The drug is a combined antibiotic that has anti-inflammatory and antibacterial effects. The medicine is used in ophthalmology and otolaryngology to treat infections of the eyes, ears, and nose. Sofradex drops, in addition to two antibiotics, contain synthetic hormones, so they must be prescribed by a doctor. Reviews from experts confirm the effectiveness of the drug against infections caused by Staphylococcus aureus.
Compound
The drug is available in the form of drops. The main active ingredient is framycetin. This antibiotic is active against gram-positive and gram-negative microorganisms. To enhance the bacteriostatic effect of the drug, gramicidin is added to the composition. It enhances the effect of framycetin. The drops are colorless. When opening the bottle, a pronounced odor of phenylethyl alcohol appears. Full composition of 1 ml Sofradex:

Pharmacodynamics and pharmacokinetics
The drug contains antibiotics of the tyrothricine and aminoglycoside groups. Sofradex is effective against Staphylococcus aureus, Escherichia coli, Proteus, dysentery bacillus, gram-positive and gram-negative microorganisms. It is not recommended to use this medicine for the treatment of infectious inflammatory diseases caused by streptococci. The drug does not affect anaerobic flora, fungi and viruses.
Resistance of microorganisms to Sofradex develops slowly. Dexamethasone prevents further development of inflammation by inhibiting the release of humoral and cellular mediators, reducing the permeability of small vessels and inhibiting the migration of mast cells. When in contact with the eyes, the solution reduces pain, burning, and sensitivity to light. When instilled into the ear canals, the medicine relieves the patient of the symptoms of otitis externa.
Indications for use
Sofradex is prescribed for allergic and inflammatory diseases of the ears and eyes. The medicine can be used to treat keratitis that is not accompanied by damage to the epithelium. During therapy, the patient must be under the supervision of a specialist, because the drug affects the pressure inside small vessels. The product is used for bacterial diseases of the anterior segment of the eye and for infected eczema of the skin of the eyelids. Indications for use of Sofradex are as follows:
- otitis of the external ear of chronic and acute forms;
- inflammation of the opaque membrane of the eye;
- inflammation of the iris;
- allergic conjunctivitis;
- iridocyclitis.

- phenylethyl and ethyl alcohol;
- twin 80;
- distilled water.
It is also worth noting that Sofradex is a medication that belongs to the group of antibiotics. This must be taken into account at the stage of choosing a treatment method for a particular disease.
Indications for use in children and adults
When are Sofradex eye and ear drops prescribed? The instructions for use indicate that this antibiotic is prescribed by a doctor immediately after making an accurate diagnosis and determining the degree of sensitivity of pathogenic organisms to antibiotics from the aminoglycoside category. This medicine effectively treats inflammatory processes that occur in the organs of hearing and vision.
Thus, this drug is taken in a number of the following cases:
- If you have blepharitis, which is an infection of the eyelids. The main causative agent of this disease is Staphylococcus aureus. A feature of this disease is long-term and difficult therapy due to the high resistance of staphylococci to many antibiotics.
- Bacterial conjunctivitis is a lesion of the mucous membrane of the eyes. The main symptoms are purulent discharge that prevents the eyes from opening. In this case, swelling may develop along with severe itching and acute pain in the organs of vision.
- For keratitis, that is, in the presence of clouding, ulceration and pain in the eyes, which contribute to the formation of a cataract or decreased visual acuity in the absence of adequate drug treatment.
- With anterior uveitis. This is a very common disease, which is characterized by a pathological change in the color of the iris, and, in addition, constriction of the pupils and turbidity of the vitreous, which leads to a sharp decrease in visual acuity in people.
- With scleritis, which is an inflammatory process of the deep layers of the sclera. This pathology usually develops as an independent disease, but it can also be a consequence of anterior uveitis. If left untreated, it leads to irreversible vision loss.
- Against the background of episcleritis. This disease is completely similar to the previous one, with the only exception that during its development only the outer layers of the membranes of the eye are affected.
- With eczema of the eyelid, which is a neuroallergic pathological condition of the skin around the eyes, against which extensive redness is observed along with itching and polymorphic rashes on the eyelids. When are ear drops prescribed? You can find information about this in the instructions for use for Sofradex.
- Against the background of acute and chronic forms of bacterial otitis, which is an inflammation of the external and, in addition, the middle auditory canal. According to statistics, eighty percent of children under the age of three have experienced otitis media at least once. Symptoms include purulent exudates along with cutting pain in the ear and shooting sensations. An increase in temperature is also possible. This disease can lead to complete hearing loss.
Also, in accordance with the instructions for use, Sofradex can also be instilled into the nose.
Indications for use of Sofradex drops
The drug Sofradex is prescribed to patients with diseases of bacterial origin with established sensitivity of pathogenic microflora to its components. It is used for the following diseases affecting the organs of hearing and vision:
- blepharitis. Bacterial infection of the eyelids, most often caused by Staphylococcus aureus,
- conjunctivitis of bacterial origin. Accompanied by damage to the mucous membrane of the eyes,
- keratitis A disease characterized by inflammation of the eye corneas, accompanied by pain and redness of the mucous membrane,
- anterior uveitis. It is characterized by the appearance of an inflammatory process, which is localized on the choroid of the eye,
- scleritis A disease characterized by inflammation of the sclera, which, if untreated, leads to loss of vision,
- episcleritis. A pathological condition accompanied by an infectious lesion of the outer layers of the eye membrane,
- eczema of the eyelids. A pathological condition that affects the skin around the eyes, accompanied by redness, itching and rashes,
- bacterial otitis. Ear disease, which can be acute or chronic.
Medicine Genodex
Another medication that has a combined composition and is used to treat superficial infections of the organs of vision and hearing is the drug “Genodex”. It is prescribed to adults and children from the age of three, and the dosage of the medicine does not depend on age.
The main active ingredients of the drug are: dexamethasone, polymyxin B and chloramphenicol. The first has an antihistamine and anti-inflammatory effect, the other two are antibiotics that fight pathogens.
Like the drug Sofradex, analogues containing dexamethasone have a lot of contraindications. The first of these is age - it is not prescribed to children under 3 years of age. It is strictly prohibited to take the medication for infectious tuberculous lesions of the organs of hearing and vision, lichen, chicken pox, diseases of the cornea with epithelial defects, trachoma, glaucoma and some others.
Composition and release form
A transparent solution with a faint alcohol odor is available in dark glass dropper bottles (polyethylene nozzle) with a volume of 5 ml. The medicine is packaged in a cardboard box and comes with an instruction leaflet.
Its formula includes 3 main components and several excipients. Basic:
- Gramicidin is an antibiotic.
- Dexamethasone is a substance belonging to the group of glucocorticosteroids.
- Framycetin sulfate is a substance from the group of aminoglycosides.
Also included:
- lithium chloride;
- ethanol;
- sodium citrate.
Do not combine with other medications containing antibiotics. If complex therapy is prescribed, then a break of 15-30 minutes is observed between instillations.
Measuring corneal thickness - eye pachymetry.

Instructions for use
This antibiotic is prescribed for infection of the organs of vision and hearing; the ointment is placed in the conjunctival sac and in the ear canal, 1-2 times a day. The pharmacological composition of both drugs is absolutely the same. Before using the drops, you need to hold them in your hand to warm them up. Dropped into the eyes, pain, burning, tearing and fear of light are reduced.
When treating the eyes, the drug is instilled 1-2 drops into the retracted eyelid every 4 hours. An eye disease called conjunctivitis often occurs in childhood. With this disease, the mucous membrane of the eye becomes inflamed, swelling, swelling of the eyelids, itching, and redness appear. In children under one year of age, the presence of such symptoms indicates an immediate visit to the doctor; if the illness lasts more than two days, with symptoms of photophobia, pain in the eyes, with deterioration of vision, if a blister appears on the skin of the upper eyelid, you should consult an ophthalmologist or pediatrician.
The cause of the disease is viruses, bacteria and allergies. With viral conjunctivitis, the body itself forms immunity in 5 - 7 days, and the disease goes away on its own. Bacterial conjunctivitis is treated with antibiotics, while allergic conjunctivitis requires eliminating the allergen.
You can distinguish the symptoms of conjunctivitis by analyzing previous events. During a virus, it is very effective to rinse your nose and eyes with saline solution to avoid contracting the infection through the mucous membrane. You can make this solution yourself, one teaspoon of salt per liter of water. In case of complicated infection, the product is applied every hour.
For ear disease, the drug is instilled into the ear pulled upward and back 3-4 times a day, 2-3 drops, after which the ear is plugged with cotton wool, or a gauze swab moistened with the product is placed in the ear canal. Instillation into sore ear canals relieves redness, pain, itching and burning, and relieves congestion.
The medication can only be used for external otitis, and the eardrum must be intact. If you feel pain after instilling the medicine, this indicates damage to the membrane. Under no circumstances should you try to clean your ears with any devices; this procedure damages the eardrum, which can cause otitis media. When cleaning your ears with a cotton swab, the self-cleaning mechanism is disrupted. Before use, be sure to consult your doctor!
Instructions for use for adults and children
If there are no other recommendations, instillations are carried out according to the instructions. 1-2 drops of the drug are instilled into the conjunctival sac of the inflamed eye. The frequency of procedures depends on the degree of the disease and the severity of symptoms. Instillations can be carried out once every 4-6 hours (in mild cases) or every hour 6 or more times a day (in severe cases).
As the inflammatory process progresses, the number of instillations decreases.
The maximum duration of therapy is 1 week.
Clinical trials of Sofradex eye drops on pregnant patients have not been conducted. But given that the drug enters the blood in small quantities, can penetrate the uteroplacental barrier and be excreted in breast milk, it is not used for the treatment of pregnant and lactating women. If there is a need to use this particular remedy, it is recommended to temporarily wean the child from the breast and transfer him to a special diet according to his age.
It is not used for the treatment of newborns. For children under 7 years of age, the dosage is determined by the doctor.
You should not use contact lenses during treatment.

Peculiarities
When using the drug "Sofradex" for more than ten days, the patient increases the likelihood of superinfection as a result of the body becoming accustomed to the active microelements of the drops.
Prolonged use of the medication in the organs of vision can cause thinning of the cornea of the eye, which in most cases is the cause of its perforation. In addition, the use of the drug in medical practice (ophthalmology) for more than ten days contributes to the formation of glaucoma.
If there is severe redness of the mucous cavity of the visual organ of unknown origin, it is not recommended to use drops until certain studies have been carried out.
The structure of the medication includes the main active microelement, which belongs to the group of substances that cause toxic effects on the auditory organ and kidneys. The likelihood of a negative lesion being used increases if the drug is instilled on open wounds.
When instilling Sofradex drops, you should not touch the pipette with your fingers, as this leads to the growth of bacteria inside the bottle.
People who use the drug Sofradex to eliminate diseases of the visual organ should refrain from driving a car, since blurred vision may be observed for some time after instilling the medicine.
Medical experts consider the drug one of the most effective remedies in the treatment of otitis media. According to patient reviews, it is Sofradex that helps quickly eliminate the disease. People noted that the ear drops began to work after the first instillation.
Possible adverse reactions
In isolated cases, patients may experience various types of allergic reactions from therapy. The most common unwanted effects:
- itching;
- pain;
- tingling;
- burning;
- temporary decrease in visual acuity;
- fogginess, a veil before the eyes.
Systemic side effects include:
- the development of diseases of a fungal nature against the background of microflora disorders or glaucoma (increased intraocular pressure);
- damage to the optic nerve;
- thinning of the cornea.
Mild side effects disappear within 15 minutes. During this period, it is better to refrain from driving vehicles and operating machinery. If severe complications are suspected, consultation with a specialist and discontinuation of the drug is essential.
An expensive and effective drug for moisturizing and protecting the cornea is Restasis eye drops.
A common side effect is burning in the eyes.

Indications for use
The combined medication “Sofradex” has found wide application both in ophthalmology and in ENT practice. Drops are often prescribed by otolaryngologists for various forms of otitis. The components included in the product effectively eliminate itching, quickly neutralize pain, relieve excessive reaction to light, eliminate lacrimation and alleviate other symptoms of eye inflammation.
In ophthalmological practice, Sofradex is often prescribed. Eye drops have the following indications for use:
- barley;
- eczema of the eyelids;
- blepharitis, when the inflammatory process affects the lower edge of the eyelids;
- conjunctivitis of various types;
- keratitis;
- iridocyclitis;
- allergic manifestations of a seasonal nature, for example, a reaction to pollen, as well as medications, food, etc.
The medicine has a visible bactericidal effect and also acts as an antiallergic and anti-inflammatory agent.
Indications for the use of Sofradex ear drops
Doctors today increasingly prescribe the drug Sofradex to their patients in the treatment of any inflammatory disease of the mucous membranes. In addition to being used to treat ear diseases, you can even drip them for inflammation of the mucous membranes of the eyes. The main indication for the use of Sofradex remains otitis media.
Moreover, it can be external, middle and internal. In addition, drops can cope with the disease at the acute and chronic stages. The drug can also be used in the treatment of conjunctivitis, eczema of the outer eyelid, adenoids and other diseases of the ears, eyes and nasopharynx.
When developing the drug, three main components were used:
- Framycetin sulfate . It has a pronounced antibacterial effect, affecting gram-positive and gram-negative bacteria. It can even destroy Staphylococcus aureus. The main advantage of this component remains that bacteria develop resistance to it extremely rarely. As a result, the drug is considered one of the most effective in the treatment of otitis media.
- Gramicidin . It has a bacteriostatic and bactericidal effect. Due to it, the effect of framycetin is enhanced and expanded.
- Dexamethasone . This is a glucocorticosteroid that has a pronounced antiallergic and anti-inflammatory effect. This component is characterized by the ability to stop the inflammatory process.
And although Sofradex effectively copes with otitis media, the drug has a number of contraindications, which include:
- individual intolerance to certain components of the drug;
- infectious process of viral or fungal origin;
- swelling of the eardrum;
- pregnancy and lactation;
- newborn children.
Drops are prescribed with extreme caution when treating otitis media in young children.
Overdose
An overdose of Sofradex can occur in two cases:
- with long-term treatment;
- with a single large dose.
If the treatment is delayed (regardless of the method of administration), the child may experience allergic reactions, including increased intraocular pressure, itching and burning at the injection site - the eyes, nose or ear canal.
If a child accidentally swallows no more than 10 ml of medication, then usually overdose symptoms do not appear. However, it is better for parents to take certain measures:
- firstly, report the incident to the doctor;
- secondly, give the child “Smecta”, “” or activated carbon, as well as any absorbent drug and monitor his condition.
Contraindications
The following factors are considered contraindications to the use of Sofradex:
- hypersensitivity to at least one of the components of the product;
- tuberculosis;
- trachoma;
- traumatic damage to the mucous membrane of the eyes, eardrum, ear canal and other parts of the ear;
- serious kidney problems;
- period of pregnancy and breastfeeding;
- in children under 2 years of age;
- with glaucoma, some vascular pathologies and increased intraocular pressure.
During pregnancy
To date, no experiments have been conducted to establish the negative effect of this antibiotic on the course of pregnancy. But it is known that this medicine can diffuse across the placental barrier and easily pass into breast milk. In this regard, pregnant women and women who are breastfeeding are prohibited from instilling Sofradex into their eyes and ears. Instructions for use must be followed.
If there is an urgent need, breastfeeding must be suspended. It is also worth mentioning that the active ingredients included in the composition have an extremely negative effect on the child’s intestinal microflora and can lead to the formation of serious pathological processes.
Analogs
The medicine has no analogues for the active substance. In terms of effect, close analogues are:
- Tobradex. It is based on torbamycin and dexamethasone. The main characteristics are the same.
- Dexon. The active ingredients are neomycin sulfate and dexamethasone sodium phosphate. Not prescribed for children under 18 years of age.
- Phloxal. The active ingredient is ofloxacin. For the treatment of adults and children of different age categories, the same dosage of the drug is used, but the number of instillations is determined individually by the doctor.
- Dexa-Gentamicin. Eliminates symptoms and fights the cause of inflammation.
- Albucid. It is based on sulfacetamide, its concentration is different for children and adults. You must read the instructions for use of Albucid drops.
- Garazon. The main components are betamethasone and gentamicin. Use with caution in diabetes mellitus and severe stage of myopathy. Not used to treat preschool children.
After opening, the bottle is stored in the refrigerator for no more than 28 days. If sediment appears or the contents become cloudy, the product is discarded.
A small yellowish formation that causes discomfort when blinking is a pinguecula.

How to treat ears with Sofradex?
According to the manufacturer's official instructions, Sofradex, as prescribed by a doctor, is instilled into the ears of patients over the age of one year, 2-3 drops in each ear 3-4 times a day.
The duration of treatment should not exceed 7 days, since exceeding the specified period can cause pathogen resistance to antibiotics and severe inflammation. The condition for using the drug for otitis is the integrity of the eardrum. Rules for instilling the drug Sofradex into the ears of a child:
- Before the procedure, you need to clean the baby’s ears with a cotton swab;
- lay the child on his side;
- gently pull the auricle back and up to open the ear canal;
- the medicine should be at room temperature;
- drop the required dose of the drug into the ear;
- close the ear passage with a dry cotton swab;
- wait 5 minutes;
- turn the baby over to the other side;
- repeat the procedure with the second ear;
- if the baby does not allow instillation, and the inflammation is localized in the outer segment, then instead of instillation it is allowed to place a cotton swab soaked in the medicine overnight.
Why Sofradex disappeared from pharmacies
Lately, Sofradex drops have been out of stock in pharmacies. This is due to the fact that the state drug service for registration of pharmacological drugs has banned its sale, use and storage. This decision was made due to the inability to control the quality of the drug.
Business entities have been instructed to check the availability of the drug Sofradex in pharmacies by the Indian manufacturer Aventis Pharma Ltd. If such batches are found, they must be returned to the supplier or disposed of.
Similar ear products
An analogue of the drug Sofradex is Polydex ear drops, which are effectively used to treat otitis media of various etiologies. Polydexa is drops for the treatment of inflammatory or infectious diseases of the ears. Drops are produced in bottles with the main components - neomycin, dexamethasone and polymyxin B.
This combination medication is used in the treatment of external and internal otitis, eczema in the ear canal. If there is no perforation or defect in the eardrum, then Polydkeksa can be used to treat otitis media.
Description of the drug
It is prohibited to use Polydex in the treatment of:
- damage or perforation of the tympanic septum, which was formed due to infection or injury;
- intolerance by the patient to the components of the drug.
During pregnancy, the drug is prescribed with great caution and in extreme cases. During lactation, you can use the medicine in dosages prescribed by your doctor.
Place 1-5 drops of medication into each ear canal twice a day. The therapeutic course is prescribed individually for each patient, based on the pathologies and their severity. The standard medication course lasts 6-10 days. Before using the drops, they must be warmed to body temperature in the hand.
Adverse reactions of the body appear infrequently, and if the medicine is used strictly according to the prescribed therapeutic dosages, then no manifestations occur. There may only be itching and a slight burning sensation near the ear canal, where the procedure for introducing drops into the ear is carried out.
If the medication is used for a defect of the tympanic septum, then toxic damage to the auditory canal, inner ear and vestibular apparatus may occur.
Composition of Sofradex eye and ear drops
Sofradex is a time-tested and popular medicine among pediatric otolaryngologists. Its widespread use is explained by the successful combination of active ingredients (per 1 ml of the drug):
- framycetin sulfate (5 mg) - an antibiotic from the aminoglycoside group, is active against a wide range of bacterial pathogens, including gram-positive bacteria that are resistant to penicillins, destroys the cell membranes of microorganisms and disorganizes their metabolic processes, is used only locally;
- gramicidin (0.05 mg) is a local antibiotic, active against staphylococci, pneumococci, meningococci and other gram-positive bacteria, has a bacteriostatic effect (inhibits the ability of pathogenic microorganisms to reproduce) and a bactericidal effect, destroys the membranes of pathogenic cells, causing disruption of their water metabolism ( osmosis of the cell);
- dexamethasone (0.5 mg) is a hormone from the group of corticosteroids, affects inflammatory mediators, relieves swelling and allergic reactions, when used topically does not have a systemic effect, and therefore is safe for children (we recommend reading: Dexamethasone eye drops for children: instructions for application).
The drug has antimicrobial, anti-inflammatory and antihistamine effects. The medicine contains auxiliary components:
- alcohols – phenylethyl and methylated;
- surfactant (surfactant) polysorbate;
- citric acid;
- salts – lithium chloride, sodium citrate;
- purified water.
Use for children
Despite the manufacturers' warnings, pediatric practice allows the use of Sofradex in the treatment of newborns and infants, but in the absence of contraindications. For this reason, you should not use nasal drops without a doctor's advice. The potent drug is toxic and is prescribed strictly according to indications. Read more about runny nose in infants→
In children, Sofradex nasal drops are used in the complex treatment of chronic or allergic rhinitis, sinusitis and adenoids, as well as for the prevention of otitis media. Depending on the causative agent of the pathological condition, the drug is administered into the nasal cavity in pure or diluted saline. For newborns, it is permissible to insert turundas soaked in a diluted solution into the nostrils.
Treatment of children requires caution and attentiveness. It is forbidden to use the medicine for more than 3-5 days in a row and to instill undiluted medicine into the nose. A prerequisite for treatment is compliance with the maximum recommended dosage specified in the instructions for use for Sofradex nasal drops. Excessive use of the drug can cause adverse reactions.
Side effects and contraindications
Sofradex drops should not be administered into the nose of a child under three years of age. They are also prescribed to preschool children with great caution. It is prohibited for babies under one year of age to put pharmaceuticals in their nose. This comes with a risk of side effects. For similar reasons, the medicine is not recommended for pregnant women and during lactation.
Sofradex also has other contraindications. The medication should not be used for the following ailments:
- Glaucoma;
- Pulmonary tuberculosis;
- Exacerbation of herpes;
- Purulent infectious diseases.
If we talk about side effects when using the pharmaceutical drug nasally, allergic manifestations of skin rash with burning and itching were recorded. Most often, this reaction was caused by the presence of a steroid hormone in the drug.
Long-term use of the drug can lead to dysfunction of the adrenal cortex.
Analogues of this product
If you do not have the opportunity to find Sofradex eye drops, then you can focus on its analogues, which are:
DexaTobropt is an analogue of Sofradex and has anti-inflammatory and antibacterial effects. The main component of the drug is dexamethasone. Belongs to the group of broad-spectrum antibiotics.
Intended for the treatment of inflammatory processes in the eyeball, including inflammation of the cornea. 1-2 drops are instilled into each eye. To do this, you need to pull back the lower eyelid and direct the dropper to the area of the conjunctival sac.
The frequency of instillation is 2 hours. When improvement occurs, the frequency of use of the drug decreases.
Before using the drug, shake the bottle. Use under medical supervision. There are contraindications.
Garazon is an analogue of combined action. Belongs to the group of broad-spectrum antibiotics. The main active ingredients are gentamicin sulfate and betamethasone disodium phosphate.

The dosage is 1-2 drops into the affected eye three times a day. During the acute course of the disease, the recommended dosage is 2 drops, and the frequency of instillation is 2 hours.
As the patient's condition improves, the frequency of instillation of the affected eye decreases. The duration of treatment should not exceed 2 weeks
For ear diseases, the recommended dosage is 3-4 drops. If pain occurs, it is recommended to use a turunda. The duration of treatment depends on the recommendations of the attending physician.
In addition to these drugs, drug analogues that are recommended for use include: framycetin, tobrazon, tobradex, dexon.
Composition and effects
French Sofradex drops have long been known on the Russian market. The drug is intended for the treatment of otitis media, diseases of the eyes and nose, therefore it is considered a universal remedy. It is a colorless liquid with a specific odor of ethanol. Read more about nasal diseases→
Sofradex, being a combination drug, contains three active components:
- Dexamethasone. A glucocorticoid-type substance suppresses the inflammatory process and reduces capillary permeability. It has an antiallergic effect and facilitates natural breathing through the nose.
- Framycetin. An antibiotic from a series of aminoglycosides, it is especially active against staphylococci and other microorganisms that cause diseases of the ENT organs.
- Gramicidin. Antibacterial agent for topical use. Enhances the effect of framycetin, inhibiting the ability of microorganisms to reproduce.
Thanks to dexamethasone, nasal drops have a beneficial effect on the mucous membrane, eliminate inflammation, swelling and irritation, and relieve congestion. Sofradex, being an anti-allergenic drug, effectively fights allergy symptoms - itching, burning, sneezing, red eyes, lacrimation.
The antibiotics included in Sofradex give it antibacterial, anti-inflammatory and bactericidal effects.
Contraindications and side effects
Since the drops are a hormonal agent, their irrational use can cause a number of side effects. Among them:
- dryness, itching and burning of the mucous membrane;
- deterioration of the adrenal glands;
- addition of candidiasis;
- decreased local immunity.
Despite its versatility in the treatment of ENT diseases, the use of the medicine must be agreed upon with the attending physician. Following the instructions and medical recommendations greatly reduces the likelihood of side effects.
If such symptoms appear, you must immediately stop using the drug and contact an otolaryngologist to adjust the treatment.
In case of an overdose of drugs, local reactions occur:
- burning and itching of the mucous membranes of the sinuses due to their dryness;
- addition of fungal infections.
Against the background of an overdose, side effects such as disruption of the adrenal glands may develop.
Dosage and mode of application
Like any medicine, Sofradex for the nose requires studying the instructions for use. This is partly due to the fact that each disease requires an individual dosage. Typically, the course of treatment varies from 4 to 6 days, after which the symptoms of the inflammatory process completely disappear.
For adenoiditis, the instillation regimen is 2 times a day, 4 drops of the drug must be injected into each nostril. If the otolaryngologist decides to extend the course of treatment to 10 days, then the dosage is halved.
For otitis media, as well as for the purpose of its prevention, Sofradex is diluted with saline solution (1:1) before instillation into the nose. The instillation procedure is performed twice a day, 2 drops in each nostril.
For chronic rhinitis, a concentrated solution is often used. The medicine is administered 2 times a day, but the dosage is increased to 3 drops. It is worth emphasizing that instillation of drops into the nose must be carried out in a horizontal position, with the head tilted back at this time. This will allow the drug to penetrate directly into the site of infection.
It is important to note that the doctor prescribes the drug. In each specific case, the dosage and frequency of use are determined individually.
Indications and contraindications for use
The anti-inflammatory and antibacterial effect of the drug allows it to be used to treat a number of ophthalmological problems, as well as in otolaryngology to treat the following diseases:
- conjunctivitis;
- scleritis;
- uveitis;
- keratoconjunctivitis;
- iridocyclitis;
- blepharitis;
- infected eyelid eczema;
- external otitis in chronic and acute form (bactericidal).
Sofradex is prescribed strictly as prescribed by the attending physician. Independent use is strictly prohibited, as it has a number of contraindications and side effects.
Sofardex is strictly contraindicated in the following cases:
- In case of intolerance to the components of the drug.
- During the course of diseases such as tuberculosis, inflammatory processes with purulent accumulation, trachoma, viral and fungal diseases of the cornea.
- For corneal damage and glaucoma. If it is impossible to refuse the drug, consultation with a specialist is recommended before using it.
- Perforation of the eardrum.
- The period of bearing and feeding a child.
- Babies.
The use of drops for young children is permitted in very rare cases and implies complete control by the doctor over the treatment process.
When treating, it is worth remembering that the wrong dosage can cause the side effects mentioned above.
If these symptoms occur, you should immediately notify your doctor. Perhaps the drug is not suitable for your body and needs to be replaced.
Contraindications for use
Sofradex is not prescribed to children under three years of age. Pregnant and breastfeeding women should avoid nasal drops. Contraindications are also:
- herpetic manifestations;
- glaucoma;
- pulmonary tuberculosis;
- fungal and viral infections.
It is important to note that one of the components of the drug, namely framycetin, can have a toxic effect on the hearing structures. The use of Sofradex is permitted only if the patient's eardrum is not damaged. Before prescribing the drug, the otolaryngologist is obliged to check the condition of this organ.
Popular analogues
Instead of Sofradex, its substitutes are often prescribed, the basis of which is similar in the mechanism of action.
Known analogues of the product:
- Dexon. Drops are used in ophthalmology and otolaryngology. The active substance is dexamethasone. Excipient: neomycin sulfate; it is a broad-spectrum antibiotic. Drops have an anti-inflammatory, antihistamine effect. Difference from Sofradex: the medicine is not prescribed to patients under 12 years of age. For the treatment of ear diseases – from 7 years of age.
- Otinum. Ear drops based on choline salicylate. They act on otitis media as an antimicrobial and anti-inflammatory agent. Gives a slight analgesic effect. Children's age is a contraindication for use.
- Genodex. Combination type product. Used for eye and ear diseases. The main base component is dexamethasone. The drug is prescribed to children from 3 years of age. Prohibited for tuberculosis and glaucoma.
- Garazon. A combination type drug used by otolaryngologists and ophthalmologists. Prescribed for blepharitis, styes, rhinitis of various types. The main substances are betamethasone, gentamicin. According to statistics, it can cause allergic reactions. Not prescribed for children under 12 years of age.
Combination drugs help eliminate various symptoms; their use according to the instructions has an effective effect.
Composition of the pharmaceutical product
The components of the medication are:
- Antibiotic substances – gramicidin and framycetin;
- Corticosteroid – dexamethasone;
- Auxiliary components (citric acid, alcohol, sodium citrate, polysorbate, lithium chloride).
Dexamethasone, which is part of Sofradex, prevents the proliferation of virulent microorganisms and the spread of infection. In addition, it helps relieve the active inflammatory process. The effect of gramicidin and framycetin is aimed at destroying pathogenic bacteria in order to suppress inflammation.
When is it prescribed?
Drops for instillation are prescribed for several types of eye diseases:
- with conjunctivitis;
- blepharitis;
- barley;
- irite.
For instillation into the ears it is used for acute and chronic otitis. Otitis is a disease that results in inflammation of the ear.
Otitis, according to the degree of infection, is divided into external, middle and inflammation of the inner ear. Indications for use of Sofradex are otitis externa, when inflammation affects the tissues of the external meatus, eardrum and outer part of the ear.
Usually the cause of otitis is the development of an infection caused by bacteria, but sometimes they are also non-infectious. Chronic is called otitis media that lasts more than 4 weeks in a row.
Otitis media can be considered chronic if it occurs more than 4 times during the year. Otitis externa develops as a complication of acute respiratory diseases and is characterized by severe acute pain.
Rhinitis is the name given to common symptoms of inflammation of the inner linings of the nose. Infectious rhinitis is caused by bacteria or viruses. Allergic rhinitis has a different nature of formation.
Sinusitis is an inflammation of the maxillary part of the nose that occurs as a result of untreated rhinitis, as a complication of influenza, viruses, and colds. The disease begins when bacteria or viruses penetrate the maxillary sinus.
Adenoiditis is the process of growth of the nasopharyngeal tonsil; with abnormal adenoids, breathing problems are often observed. According to statistics, boys are more likely to suffer from adenoiditis.
Analogues of Sofradex used in pediatrics
Polydex ear drops (Bouchard Laboratory, France) have a similar medicinal effect; this is a combination drug that contains dexamethasone and two antibiotics (neomycin and polymyxin B).
Approved for use in children of any age. It has the same indications as Sofradex. Children are prescribed 1-2 drops in each ear 2 times a day for a week.
Neomycin, which is part of Polydexa, has an ototoxic effect, so the drug is not prescribed for purulent otitis. The duration of treatment should not exceed 7 days.
How to choose a dose
The doctor selects the dose. The instructions for use provide standard dosages for children. The product is instilled into the ear, 2-3 drops 3-4 times a day. Some otolaryngologists recommend using turundas (pieces of gauze rolled into a tube) moistened with medicine. They are inserted into the external auditory tube 4 times a day and kept until dry. The course of treatment should not exceed seven days.
The instillation procedure begins with straightening the external auditory canal. To do this, you need to pull the auricle upward. The bottle with drops must first be warmed in your hands and only then the required number of drops must be dripped into the ear. After instillation, the patient should lie on his side for 5–7 minutes so that the drops do not leak out.
Not according to the instructions: is it possible to drip Sofradex into the nose?
According to the instructions, Sofradex is intended for the treatment of eyes and ears, but many experienced pediatricians have mastered the practice of using it to treat prolonged runny nose and adenoiditis. To do this, it is diluted in saline or distilled water in a 1:1 ratio. In this case, it is safe for the nasal mucosa.
For a persistent runny nose
Many parents use medicine for children with a runny nose on their own, without consulting a specialist, not taking into account that in most cases a runny nose is caused not by a bacterial, but by a viral infection. Sofradex is ineffective against viruses. The drug can be used to treat a runny nose in the following cases:
- allergic rhinitis;
- sinusitis;
- persistent runny nose caused by staphylococcus, streptococcus or pneumococcus.
The medicine is instilled into the nose in diluted form, 4 drops three times a day for no more than 7 days. Before use, the nose must be cleared of mucus. To do this, first a solution of sea salt is instilled (Aqua Maris and others), then the child carefully blows out the mucus, and only then Sofradex is instilled.
For the treatment of adenoids
The use of the drug in the treatment of adenoids is recommended by many pediatricians, including Dr. Komarovsky. It is justified in the absence of the following contraindications:
- postoperative period;
- dermatological diseases;
- fungal infections of the adenoids;
- undergoing systemic hormonal therapy.
Enlarged adenoids are treated by instilling the drug into the nose or by inhalation using a nebulizer. Most often, doctors recommend using nasal drops for treatment for 7–10 days, 3–4 drops in each nasal opening three times a day. The duration of treatment and dose are individual, determined by the attending physician and depend on the nature of the disease. Instillation is performed in a lying position.
Possible complications caused by the drug
Complications are mainly allergic in nature. They manifest themselves in the form of irritation, burning, pain, itching, and dermatitis.
If Sofradex is instilled for a long period, intraocular pressure may increase and glaucoma may develop. This leads to a decrease in the quality of vision and loss of acuity. With long-term and regular use there is a risk of systemic effects. In this case, symptomatic treatment is carried out. If you drink no more than 10 ml of the medicine, there is a low probability of developing severe complications.
If the doctor has prescribed a course of treatment longer than one week, then IOP must be measured regularly.
If there are complications, you should consult a doctor.
Price and analogues
Price per bottle of product – 313-427 rubles. depending on the region and pharmacy chain. The cost of analogues, as a rule, is in the same price category. Drugs with a narrower effect can be purchased cheaper.
Analogues of Sofradex:
- Ophthalmeron. It has a similar list of indications for the treatment of eyes and ears. The difference is in the active substance - interferon, which is more of an auxiliary agent in treatment. Price – about 247 rubles.
- Isofra (spray) (more details in the article: “Isofra” nasal spray: instructions for use for children). Contains the same active substance - framycetin sulfate, therefore it is an antibiotic intended for the treatment of rhinitis and sinusitis caused by infectious diseases. Price – 375-395 rub.
- Gentadex. The active substance is dexamethasone sodium phosphate (see also: instructions for use of Dexamethasone drops for children). Strong immune and anti-inflammatory agent. Cost – from 120 rubles.
Ear drops from the same manufacturer will help cure your eyes, ear, and nose (see also: anti-inflammatory eye drops: list of the best drugs for children). For adenoids, the drug will also have a positive effect. Before using the product, it is important to know that it can only be used with a correct diagnosis - otherwise it will only worsen the child’s condition.

Sofradex - how will it help with otitis media?
These are ear and eye drops with antibacterial and anti-inflammatory properties. Antibacterial substances (framycetin and gramicidin) suppress the main types of bacterial infections that cause inflammatory processes in the ENT organs, but do not show activity against pathogenic viruses and fungi. The glucocorticoid hormone dexamethasone instantly relieves inflammation and swelling, as well as associated pain and itching. After instillation, the pain in the ear goes away within half an hour.
Otolaryngologists and parents love these drops for their effectiveness. But parents are often outraged that ear medicine can only be purchased with a prescription. This is due to the fact that Sofradex has clear indications and contraindications for use.
Reviews about the drug
Most parents write positive reviews about Sofradex:
Nina, 32 years old
“My son had an earache due to a runny nose. I applied Sofradex and everything went away. True, the pediatrician scolded me and said that it should not be given without the permission of the ENT specialist.”
Natasha, 28 years old
“My daughter often suffers from otitis media and I always have these drops. The doctor told me that they should not be instilled if pus is flowing from the ear, and I always watch for this.”
Sofradex drops help many children survive ear pain and recover. But it should be remembered that this medicine cannot be instilled into children’s ears without an examination by an otolaryngologist.

Indications and contraindications
Sofradex, being a universal remedy, is used as part of complex therapy and prevention:
- otitis media;
- chronic and allergic rhinitis;
- sinusitis;
- adenoiditis.
Experienced otolaryngologists often prescribe drops during the development of a runny nose to prevent otitis media. Inflammation of the hearing organs often occurs as a complication of a respiratory infection located in the nasopharynx. Therefore, the use of Sofradex as a preventive measure is completely justified.
The drug is also used for allergic rhinitis. The drug helps to cope with illness without vasoconstrictor medications, quickly relieves symptoms, eliminates nasal congestion and excessive secretion.
The antibacterial properties of Sofradex help improve the condition of patients with sinusitis and adenoiditis. The drug, of course, cannot completely cure such serious diseases, but it can reduce the manifestations of the disease and prevent complications. Sofradex, being a strong antibacterial and hormonal medicine, has a number of contraindications that you need to heed:
- individual intolerance to the components of the drug;
- fungal or viral infections of the nasal cavity;
- tuberculosis;
- perforation of the eardrum;
- infancy;
- period of pregnancy and lactation.
Ear drops for the treatment of otitis media
These ear drops, like the products of the first group, contain various antibiotics. But we will consider two drugs from the group of fluoroquinolones, which can be used for various types of purulent otitis - these are the popular ear drops Normax and Dancil.
They are represented by different generations of antimicrobial active ingredients, help block the proliferation of microbial flora and provide a bactericidal effect.
Normax (norfloxacin)
Normax is a monocomponent substance, its active ingredient is norfloxacin from the group of fluoroquinolones. In addition to otiatrics, this remedy is used in ophthalmology; it can be instilled into the eyes. It has the best effect when the existence of gram-negative bacteria is confirmed. Normax drops do not cope so well with gram-positive microbes.
Normax is used for inflammation of the middle and outer ear, as well as for eustachitis, or inflammation of the auditory tube. The drug is prescribed 1-2 drops 3 to 4 times a day. At the very beginning of treatment, in case of acute pain, you can drip two drops every 2 or 3 hours on the first day. Normax is produced by the Indian pharmaceutical company Ipka; one small bottle of 5 ml costs 170 rubles.
Advantages and disadvantages
Normax is widely available in pharmacies, and its price is quite affordable, but it is not used in children, lactating and pregnant women. For a more pronounced effect, it is necessary, in addition to ear drops, to use tableted antimicrobial drugs, and in severe cases, to use them in the form of intramuscular injections.
The drug causes side effects, local allergies, dizziness or insomnia, so the first use is preferably carried out after identification of the culture based on bacteriological examination of discharge from the ear and after consultation with an ENT doctor.
Dancil (ofloxacin)
Dancil is the next generation of fluoroquinolones. In tablets it is known as Tarivid or Zanotsin. But the drug is produced in the form of ear drops by the Indian company Sentiss Pharma. The range of applications is very wide - it is prescribed for purulent and non-purulent otitis, but its unique property is the ability to use it without fear in case of perforation of the eardrum, which cannot be done with almost any other representatives of ear drops.
It can be used in children after reaching the age of twelve and in adults. Medium regimen – 10 drops into the sore ear, up to 2 times a day. The course of treatment should not exceed 14 days. One bottle of the same volume, 5 ml, costs inexpensively, about 160 rubles.
Advantages and disadvantages
The most important advantage of Dancil is the ability to use it for perforation of the eardrum, as well as after surgery, when the integrity of the eardrum is damaged. The medicine is used in ophthalmology, therefore it is very useful for blepharitis, conjunctivitis and inflammation of the lacrimal glands.
The drug is often found in pharmacies, is well tolerated and has a low cost, so it can be used by low-income people.
Available Substitutes
On the pharmaceutical market there are equivalent substitutes for Sofradex eye drops. When choosing a drug, ophthalmologists take into account the diagnosis, the extent of the pathological focus, the presence of concomitant diseases, and the patient’s age.
Dexon
Dexona - combined drops for the treatment of inflammatory processes in the organs of vision and hearing. They contain the hormone dexamthasone and neomycin (an antibiotic).

- eye damage due to tuberculosis.
Adverse reactions develop infrequently in the form of discomfort in the eyes and local allergic manifestations. If an overdose occurs, the gas is washed with warm water to avoid negative consequences.
The average price of a 5 ml bottle is 121 rubles. This is a cheap analogue of Sofradex drops.
Genodex
Jenodex - eye and ear drops based on dexamethasone and antimicrobial agents (polymyxin, chloramphenicol). The Sofradex analogue is used for adults and children over 3 years of age. The solution is well tolerated by patients; no cases of overdose have been recorded.

The drug is toxic and often causes serious complications:
- deposits on the cornea, decreased quality of vision, dryness, peeling, formation of scales on the eyelids, double vision;
- headaches, migraines;
- profuse rhinorrhea (nasal discharge);
- general increase in body temperature.
The duration of therapy should not exceed 7 days. The average cost of drops is 404 rubles.
Combinil Duo is strictly prohibited for use in pediatric practice. In children, the drug causes cardiovascular collapse, severe anaphylactic reactions, swelling of the larynx, even stopping breathing.
Aprolate
Aprolate is an analogue of Sofradex in the form of a suspension based on oxytetracycline, polymyxin, hydrocortisone. Used for bacterial conjunctivitis, marginal inflammation of the eyelids (marginal blepharitis), keratitis, dacryocystitis.
The drug can cause visual disturbances, local allergies, increased intraocular pressure, and secondary fungal infection. Therefore, it is extremely rarely prescribed to children, only in the absence of alternative treatment methods.
Drops are contraindicated for viral infections (chickenpox, herpes), fungus.
Garazon
Garazon is a solution containing gentamicin, betamethasone. The medicine is indicated for diseases of the superficial structures of the eye, except for viral and fungal infections. Prescribed to patients over 12 years of age.
The treatment regimen begins with a loading (maximum) dose and gradually decreases as the condition improves. With a single overdose, no side effects occur. If you systematically violate the prescribed regimen, symptoms of hypercortisolism (excess of adrenal hormones) may appear - rounding of the face, blush on the cheeks, decrease in muscle mass.
Indications for use
Sofradex nasal drops are indicated for the following conditions:
- otitis;
- sinusitis;

- allergic and chronic rhinitis;
- inflammation of the edges of the eyelids (blepharitis);

- scleritis and so on.
Sofradex drops are used mainly for the treatment of a runny nose.
Sofradex in the nose is used both for the treatment of already developed pathologies and as a prophylactic agent. Nasal drops are often prescribed for a runny nose to prevent complications such as otitis media.
The drug Sofradex is effectively used for allergic and chronic rhinitis. With its help, you can quickly get rid of a runny nose and accompanying signs of inflammation.
Due to its antibacterial effect, Sofradex in the nose is prescribed for adenoiditis and sinusitis. The drug is not an absolute panacea for these diseases, but it can significantly alleviate symptoms and prevent the development of complications.
Analogs of the drug and reviews about it
If the pharmacy does not have Sofradex, you can purchase a drug with an antibacterial effect:
- Albucid eye drops (also used to treat nasal diseases in children);
- Polydex nasal spray with phenylephrine
- Isofra nasal spray.
According to reviews from otolaryngologists and parents, the nasal use of Sofradex for children is quite effective if the cause of the disease is correctly identified and the instructions are followed strictly. One of the disadvantages is the short shelf life of an open bottle. If you don't use it up in a month, you have to throw it away.
Sofradex can be used in the form of nasal drops only with the permission of a doctor. In this case, you must strictly follow the dosage and doctor’s recommendations so that the product works effectively and without side effects.
