A modern drug from the subgroup of ACE inhibitors with a pronounced hypotensive effect is Prestarium tablets. What does the medicine help with? Composition and release form

Composition, active substance

The composition includes the active ingredient perindopril erbumine, and is available in tablets of 2, 4, and 8 mg. For human convenience, depending on the content of the active component, the pills are colored in different colors:

  • white - 2 mg have a rounded biconvex shape;
  • light green - 4 mg (Prestarium 4 mg has round outlines);
  • green - corresponds to 8 mg of the drug.

In addition to the main active ingredient it contains:

  • lactose monohydrate;
  • silicon dioxide;
  • magnesium stearate.

The drug Prestarium comes in tablet form only.

The drug is sold in pharmacies only with a prescription written in Latin. For Prestarium, the recipe in Latin will look like this:

Pharmacodynamics and pharmacokinetics

The medicine, according to the instructions, reduces the pressure in the capillaries of the lung tissue, leading to regression of the dilated left ventricle. The medication restores the elasticity of the walls of large vessels. Prestarium tablets stabilize the isoenzyme profile of myosin (the protein that makes up muscle contractile fibers), normalizing the functioning of the cardiovascular system, reducing preload and afterload, and calming the heart rate (HR). The drug increases regional blood supply to muscle tissue and reduces pressure in the lumen of the ventricles.

After completing the use of the medicine, withdrawal syndrome does not develop (if the instructions are followed). The maximum concentration of the active substances of the drug in the blood is observed an hour after taking the tablets. Metabolites (decomposition products) are removed from the body primarily through the kidneys and urinary system. The instructions for the drug contain data on the registration of a persistent therapeutic effect on the fourth day of treatment with tablets.

About the drug

Prestarium is a drug intended to lower blood pressure. The medication has a wide range of actions.

The substances contained in the medicine help:

  • reduce blood pressure;
  • restore the elasticity of large arteries;
  • narrow large (arterial) vessels;
  • restore cardiac metabolic processes;
  • reduce heart rate;
  • increase the body's resistance to physical activity;
  • improve blood circulation;
  • reduce the size of the left ventricle increased due to disease.

The substances contained in the drug reach their peak effect after approximately 4-6 hours, and the pressure is maintained at a normal level for the patient for 24 hours after administration. The pressure stabilizes, and unpleasant symptoms weaken or completely disappear after stable use of Prestarium for 1 month.

Indications for use of Prestarium

Arterial hypertension is a common vascular pathology. This serious disease requires immediate and thorough treatment. High blood pressure threatens serious complications that can lead to severe deterioration in health and even cause death. If your health worsens, a person should consult a doctor who will prescribe antihypertensive drugs. Prestarium is one of the suitable drugs. Indications for use of the medicine, according to the instructions, are:

  • chronic heart failure;
  • hypertension;
  • chronic coronary heart disease (in order to reduce the risk of stroke, myocardial infarction and other complications);
  • prevention of recurrent stroke (tablets are prescribed in combination with Indapamide).

Instructions for use


The drug Prestarium is prescribed exclusively for oral use.
The dosage of this drug is selected individually and depends on the patient’s condition and his blood pressure readings.

Prestarium is often combined with antihypertensive drugs of other classes. Its starting dose in such cases is usually 5 milligrams. After a month of treatment with this drug, the dosage can be increased to 10 milligrams.

During the use of Prestarium, arterial hypotension may occur - this manifestation is possible if the patient simultaneously uses diuretics. For this category of people, care must be taken during treatment, because they may have a risk of water or salt deficiency in the body.


If possible, you should stop taking diuretics, but this should be done at least three days before starting treatment with Prestarium.

For hypertensive patients who cannot stop taking diuretics, treatment may be prescribed, but the initial dosage will be 2.5 milligrams per day.

A further increase in the amount of the drug is possible, however, it is necessary to constantly pay attention to blood pressure. If necessary, combined therapy with a diuretic can be resumed.

The tablets must be taken once a day in the morning.

For elderly patients, treatment with this medication is prescribed with an initial dosage of 2.5 milligrams per day. But with good indicators, it can be increased to 5 milligrams after a thirty-day period from the start of treatment, and then to 10 milligrams, but the functioning of the kidneys will be taken into account.


For patients with heart failure, the use of the drug is permitted with extreme caution and under close medical supervision.

The starting dosage of the drug is 2.5 milligrams per day in the morning. In the future, after consultation with a doctor and with good tolerance, the dose of the drug can be increased to 5 milligrams 1 time per day.

The amount of medication in this case is determined individually and depends on the clinical effectiveness for each patient. To prevent recurrent stroke in patients with cerebrovascular diseases, the initial dose of this drug is 2.5 milligrams.

Apply once a day in the morning. After 14 days, it can be increased to 5 milligrams with the same dosage schedule.

Directions for use and dosage

According to the instructions for use, the medication should be taken 1 tablet per day before breakfast. If you forget to take a dose, take the medicine before your next meal of the day. The medication should be swallowed whole with a small amount of water (without chewing or crushing in other ways). The dispersant tablet should be placed on the tongue and held until it breaks up into several fragments, then swallowed with saliva. The dosage of the medicine is selected for each patient individually, based on indications, blood pressure level, etc.

Prestarium A

Instructions for use of Prestarium A suggest the use of tablets both for monotherapy and as part of systemic treatment. It is recommended to start taking it with a dosage of 5 mg per day, after which, if necessary, the amount of medication is increased to a dose of 10 mg. With severe constriction of the renal vessels, insufficient volume of circulating blood, decompensated heart failure, reduced levels of electrolytes (calcium, potassium, chlorine, magnesium or sodium), after taking the first tablet, blood pressure may drop sharply.

The maximum risk of a decrease in blood pressure at the beginning of treatment is observed in patients simultaneously taking diuretics (diuretics). Patients with the listed pathologies, according to the instructions for use, should take 2.5 mg per day at the beginning of therapy, and only a week later, after the body gets used to the effect of the medicine, increase the dosage to the required one. It is allowed to bring the latter to the maximum dose (10 mg) no earlier than a month after the start of treatment.

For heart failure, 2.5 mg tablets are used in combination with potassium-sparing diuretics. After 2 weeks, if the medication is well tolerated, the dosage is increased to a daily dose of 5 mg, and it remains this way until the end of the course of therapy. Patients who have been diagnosed with electrolyte deficiency or anemia along with heart failure should have these conditions corrected before using diuretics along with Prestarium A.

When repeating a course or for prophylactic purposes, the tablets, according to the instructions, are taken together with Indapamide. In the first two days they drink only Prestarium A at a dosage of 2.5 mg, and after that they begin to supplement the drug with Indapamide, switching to tablets of 5 mg. To reduce the risk of cardiovascular complications during ischemia, the drug Prestarium is started at 5 mg per day. After 14 days, the dosage is increased to 10 mg (provided the medication is well tolerated by the patient).

Bi-Prestarium

For the treatment of primary hypertension, according to the instructions for use of the drug, 4 mg of Bi-Prestarium per day is indicated. If there is no expected effect, the dosage is increased to 8 mg. If the patient has renovascular hypertension, in which narrowing of the renal vessels occurs, it is recommended to use 2 mg of the drug per day. The dosage can be increased to 8 mg if the patient's condition requires it, but this decision can be made solely by the doctor.

Treatment of heart failure is carried out with Bi-Prestarium in combination with a potassium-sparing diuretic. It is recommended to start using the first one at a daily dosage of 2 mg, and after 1-2 days increase it to 4 mg. To avoid recurrent stroke in people with cerebrovascular pathologies, tablets are taken at 2 mg per day. This dosage is maintained for 2 weeks, after which Indapamide is added to the drug and, if necessary, the amount of Bi-Prestarium is increased to 4 mg.

Dosage

The tablets are taken once a day, on an empty stomach. The initial dose is 1 mg. Subsequently, the dosage may be increased. For congestive heart failure, the daily dose can be increased to 4 mg per day. The maximum dosage cannot exceed 8 mg.


The course of treatment depends on the individual characteristics of the patient’s body, as well as the form of the disease. For chronic heart failure, tablets are taken continuously. If renal function is impaired, dosage adjustment is required.

If the use of Prestarium is necessary to reduce the risk of cardiovascular complications in people who have previously experienced a myocardial infarction or coronary revascularization, unstable angina is possible during the first month of taking the tablets. To continue therapy for a patient, the physician should evaluate the benefit-risk ratio. When using the medicine, it is important to take into account that this can provoke a sharp decrease in blood pressure. Symptomatic hypotension often occurs in people with uncomplicated hypertension.

A high risk of lowering blood pressure appears against the background of low circulating blood volume (this may be a consequence of taking diuretics, following a salt-free diet, diarrhea, vomiting, blood dialysis, etc.). The appearance of symptoms of hypotension is sometimes observed in heart failure. For patients with this problem, it is important to carefully monitor blood pressure, kidney function, and potassium levels in the blood. If arterial hypotension develops, the patient is transferred to the supine position, raising his legs. It is tolerated quite well by the elderly category of the population, but excretion from the body occurs at a slow pace. Difficulties with the rapid elimination of metabolic substances are also observed in individuals with renal and hepatic insufficiency. These factors influence the decision about the dosage strength of the antihypertensive drug, but are not contraindications.

Co-perineva (Russian generic)

The only indication for this combined form of Perindopril with Indapamide, given in the official instructions for use, is essential or primary hypertension.

The presence of a diuretic limits the range of prescriptions for patients with impaired renal function, since the load on the renal system increases.

Medical sources repeatedly mention that the combination of Perindopril with Indapamide gives a positive result in the prevention of recurrent stroke. It is not recommended to combine the drug with ACE inhibitors and sartans, since the mechanism of action of such drugs largely coincides, and the meaning of systemic therapy lies in its ability to influence various components of the pathology. The use of tablets together with potassium salts and potassium-sparing diuretics such as Spironolactone, Amiloride, Triamterene is prohibited. The essence of pharmacological action

The active component Perindopril belongs to the group of angiotensin-converting enzyme inhibitors (hereinafter referred to as ACE inhibitors). The drug is characterized by the property of expanding the vascular lumen and, as a consequence of the first, producing a hypotensive effect.

The ultimate goal of Perindopril is to reduce the amount of the vasoconstrictor hormone angiotensin II in the blood, an increased amount of which causes high blood pressure on the vessels. This is facilitated by slowing or inhibiting the release of the AP enzyme, which further inhibits the process of converting the hormone angiotensin I into angiotensin II.

Contraindications

There are two types of prohibitions for using Prestarium - absolute and relative. The first ones include the following:

  • Quincke's edema;
  • hereditary or acquired angioedema;
  • glucose-galactose malabsorption syndrome;
  • lactase deficiency, lactose intolerance;
  • diabetes mellitus, renal dysfunction (when treated with Prestarium with Aliskiren);
  • pregnancy, breastfeeding;
  • age up to 18 years.

Diseases or conditions in the presence of which you need to exercise caution when using Prestarium are relative contraindications. The instructions indicate the following factors when taking the medication is undesirable:

  • reduced circulating blood volume caused by taking diuretics;
  • desensitizing therapy (antiallergic treatment);
  • anemia associated with taking diuretics and other factors;
  • bilateral renal artery stenosis;
  • cerebrovascular pathologies;
  • systemic connective tissue diseases;
  • angina pectoris;
  • renal, heart failure;
  • hyperkalemia;
  • condition after kidney transplantation, the presence of only one of them;
  • hemodialysis, etc.

Possibility of overdose

An overdose occurs if the individually selected dosage for Prestarium is exceeded. This happens if the patient does not take the pill on time, and the next day takes a double dose. The instructions for use indicate the following signs of overdose:

  • severe hypotension;
  • dizziness and weakness;
  • feeling of heartbeat;
