What does it consist of and how does it work?

The main active ingredient of Mydocalm is tolperisone hydrochloride. It is classified as a muscle relaxant.

Mydocalm tablets for osteochondrosis contain the following additional components:

  • citric and octadecanoic acid,
  • corn starch,

  • titanium dioxide,
  • cellulose,
  • silica,
  • "Macrogol 6000"
  • dye.

The use of Mydocalm for osteochondrosis does not have a negative effect on the cerebral cortex. It can be used for a long time, since the medicine is not addictive.

"Mydocalm" is an additional remedy to the main treatment of chondrosis. It does not interact with other drugs and does not affect their effectiveness.

Substitutes and analogues

Some medications have the same composition or have similar therapeutic effects. Mydocalm substitutes are available in the form of an injection solution and tablets. The cost of analogues is 180−200 rubles.

In the treatment of degenerative joint disorders, cervical chondrosis, neuralgia and neuropathy, a complex of drugs is used: Mydocalm, Milgamma and Movalis. Thanks to the active substances, tissue metabolism is normalized, muscle spasms and hypertonicity disappear.

Analogues of Mydocalm:

  • Tolperisone.
  • Baklosan.
  • Tizalud.
  • Miolgin.
  • Sirdalud.
  • Tolperil.
  • Tolisor.
  • Tizanidine.
  • Calmirex.
  • Baclofen.

A complete analogue of Mydocalm is Tolperisone, which contains the same component, but is distinguished by the absence of lidocaine. The drug has no anesthetic effect. The patient will only feel muscle relaxation.

Tizalud contains the active ingredient tizanidine, which has properties similar to tolperisone. The drug relieves muscle spasms in pathologies of the musculoskeletal system and spinal cord, convulsions, and restores impaired cerebral circulation after a stroke. The cost of the drug is in the range of 140−300 rubles.

Miolgin contains paracetamol and chloroxazone. The drug is effective for sprains, myositis, migraines and neurological disorders. The patient may experience side effects: shortness of breath, rash on the body, abnormal bowel movements, nausea or vomiting.

As a replacement, the doctor prescribes another muscle relaxant, for example, based on tizanidine. A group of these drugs is represented by Sirdalud. Its main effect is due to the fact that at the level of interneurons of the spinal cord, nerve impulses are inhibited. The tone decreases and convulsions are reduced. It is used after surgery, for muscle spasms and other neurological pathologies. Contraindications include pregnancy and lactation, as well as children under one year of age.

Baclofen has similar pharmacological characteristics. It is prescribed for traumatic brain injuries, sclerosis and stroke. There is a large list of contraindications and adverse reactions. While taking the drug, the patient may experience dizziness, nervousness, hallucinations, apathy, decreased blood pressure and symptoms of depression. Only a doctor can replace the prescribed Mydocalm with a suitable analogue.

Tizanil contains tizanidine, which has a relaxing and analgesic effect. The medicine is available in 30 or 20 pcs. packaged.

The composition of Mydocalm is not toxic to humans, does not have mutagenic or carcinogenic characteristics. The main advantages are good tolerability and the ability to combine its use with therapy with non-steroidal anti-inflammatory drugs.

When to take the drug for the treatment of osteochondrosis

One of the indications for the use of Mydocalm is osteochondrosis. Doctors everywhere prescribe this medication for patients with its symptoms. At the same time, a person has doubts about the effectiveness of using Mydocalm, because there are different types of diseases, but the same drug is always recommended. To dispel your doubts and understand that if any parts of the spine are affected, you can take medicine, you need to know the principle of its action.

Mydocalm is a drug produced in the form of tablets and solution for injection.

The medicine has the following properties:

  • antispasmodic (relaxes muscle tissue during spasm);
  • anesthetic (does not act directly on pain, but eliminates it through relaxation);
  • vasodilator (normalizes blood flow);
  • sedative (has a calming effect);
  • lymphatic drainage (eliminates stagnation in lymphoid tissue).

With all this, Mydocalm does not affect the functioning of the central nervous system, does not inhibit the speed of impulses and does not cause drowsiness. The use of the medication is recommended for all patients who have pain in the spine of any location.

In the treatment of spinal osteochondrosis with Mydocalm, the effect is achieved by influencing the smooth muscles of the skeleton. The medication relaxes pathologically contracting tissue and restores its original state. Due to this, blood flow is normalized, pinching of roots, nerve endings and blood vessels is eliminated.

Instructions and dosage of the drug

The instructions say that the injections are administered intramuscularly or intravenously. Intravenous injections are given very slowly. Moreover, their administration is carried out every day, one ampoule. During the day, you can use no more than 100 milligrams of the active substance. The drug is administered intramuscularly to patients one ampoule, but twice a day.

For children, the dosage of this drug is prescribed in accordance with their weight. The duration of treatment with injections is usually determined by the attending doctor in accordance with the disease, the age of the patient and his general condition.

As part of the research, experts have found that patients with liver or kidney diseases when using Mydocalm injections are much more likely to develop various undesirable reactions than those patients who do not have any problems with these organs. In this regard, such patients are prescribed an individual dosage of the drug. For people who suffer from severe kidney or liver damage, the use of Mydocalm injections is strictly prohibited.

Practical application experience

Reviews from doctors and patients who received Mydocalm injections.

My local doctor prescribed me the drug Mydocalm; I had spasticity on the left side after a stroke. To begin with, I took tablets at a dose of 50 mg, but did not feel any improvement. Then the dosage was increased to 150 mg at a time, of course, there was an effect, but I began to constantly feel sick, my appetite completely disappeared, and my blood pressure began to drop sharply.

I had to give up this form of the drug, and then the specialist prescribed me injections. Now everything is wonderful and there are no side effects and the effect is noticeable. This is a wonderful solution - injections.

I am 60 years old. I understand the changes that occur in the body - this is primarily age. But somehow you can support yourself.

My joints hurt, and lately I even began to limp. The doctor prescribed Mydocalm injections for me. At first I was even afraid to give such injections, I thought that they would fall asleep on the go. But the doctor insisted and now I continue to put them on. My condition has improved and I am not experiencing any adverse effects.

According to the results of observations, after using Mydocalm injections, a significant improvement in the patient’s condition is observed. The pain decreases, and laboratory test results return to normal.

After communicating with patients, we can often conclude that the course of therapy does not always have to be completed and the injections can be discontinued. Of course, I still recommend doing everything according to the rules.

The quality and price indicators of the medicine exceed all expected, and the product is one of the best. However, you should not engage in independent treatment; it is better to consult a specialist.

Dmitry R, general practitioner

I treat the musculoskeletal system, mostly my patients are elderly people. Mydocalm is very often present in my prescriptions - the product is effective and safe when taken in the correct dosage.

From the advice of ordinary people

The drug has its advantages and disadvantages, like any other drug.
It is not recommended to use the drug on your own, since only a doctor will be able to correctly diagnose and identify all possible side effects, based on the conclusions of other specialists or laboratory tests entered into the medical record. Using the product without a doctor’s prescription can cause numerous negative reactions, in particular swelling, which will have to be relieved with the help of diuretics, which in turn also harms the body. After the first injection, you should tell your doctor about how you feel in order to make a further decision on the use of the product.

Analogs

Drugs with similar therapeutic effects or pharmacological properties are prescribed when the patient cannot tolerate Mydocalm or the treatment has not produced results. Drugs prescribed as an alternative:

  • Sirdalud. The medicine helps well with muscle spasms. It is enough to take 1 tablet per day to improve the condition. By following the dosage recommended by the doctor, patients do not experience side effects. Sirdalud helps against spasms in the intestines that occur in the postoperative period or caused by diseases of the nervous system. If the question of which drug to choose is being decided, only a doctor can say which is better. Sirdalud and Mydocalm are similar to each other, but they have different side effects.
  • Miolgin. It is prescribed in the same cases as Mydocalm. The drug lowers temperature, relieves pain, and relaxes muscles. It is prescribed to patients over 18 years of age suffering from headaches, sprains, and nervous disorders. Recorded adverse reactions: nausea, dizziness, insomnia, allergic rashes.
  • Baclofen. This analogue is prescribed to patients with injuries and serious illnesses, including cerebral palsy, multiple sclerosis, stroke, resulting in muscle spasticity. Undesirable effects include depression, hallucinations, constipation, difficulty urinating, drowsiness, and low blood pressure.

All of these analogues are cheaper than Mydocalm, but the difference between the minimum and maximum prices can reach several hundred rubles.

Side effects

Often, adverse reactions appear in those patients who have an individual intolerance to this drug or in case of an overdose. In such cases, people may experience the following:

  • The appearance of redness in the injection area.
  • The occurrence of dizziness and headache.
  • The appearance of drowsiness or sleep disturbance.
  • The occurrence of pressure surges.
  • The patient develops anorexia.
  • The appearance of pain in the leg muscles along with muscle weakness.
  • The occurrence of digestive disorders.
  • The appearance of a feeling of fatigue along with general weakness and discomfort.
  • The presence of signs of impotence, that is, asthenia.

Much less often, patients after Mydocalm injections experience visual disturbances along with hand tremors, depression, convulsions, and impaired attention and balance. In addition, manifestations that are associated with hypersensitivity and tinnitus cannot be excluded. Sometimes after injections of this drug, patients experience increased heart rate, blood pressure drops, and angina symptoms. Among other things, cases of skin reactions in the form of rashes, redness, itching, and also excessive sweating and burning have been recorded. Mydocalm injections should be given intramuscularly by a qualified specialist.

It is worth noting that the described drug must be administered to patients by drip. If the injection rules are violated and the injections are given in a jet, then signs of arterial hypotension are likely to appear. During the study of this drug, it was possible to record isolated cases of anaphylactic shock along with a sharp decrease in heart rate and an increase in creatinine in the blood. In addition, when patients receive injections, their bone mineral density decreases and they may experience an unquenchable and unnatural feeling of thirst.

That's what the instructions say. Analogues of Mydocalm injections will be presented below.

Therapy and pain

Soreness is the most common symptom in pathologies of various types and levels, so for a long time doctors and scientists have studied the phenomenon of pain carefully and in detail. A lot of work has been carried out to determine the mechanism of pain.

The experimental data that are currently available allow us to speak with confidence about the importance of nerve endings that can perceive stimuli that harm the body. There is a relationship between the functionality of various fibers and a person’s perception of pain

Scientists know the ascending pathways, the brain structures responsible for signal processing and perception of information about pain. The modulators and mediators of the nervous system necessary for the appropriate reaction have been identified. Scientists have proven the role of gender, social environment, psychological nuances - all this determines how a person will perceive pain.

As studies have shown, among the abundance of hormones, mediators, modulators, through which the conduction of nociceptive signals is realized, special attention should be paid to amino acids, cocalcigenin, type 1 neurokinin and P-substance. Chemo-, cytokines, so-called growth factors are important

Recent discoveries in the field of molecular biology have made it possible to understand how nociceptors are excited by chemical components, heat and mechanical stress. Sodium channels have been shown to be a favorable target for an analgesic formulation. For four decades, Mydocalm has been used in medical practice, which supposedly blocks sodium channels.

Mydocalm-Richter analogues and prices

On average across the country, the price of Mydocalm-Richter ranges from 460-480 rubles.
Since the drug has a wide range of indications for use, such prices can significantly hit the family budget. Therefore, today doctors are increasingly prescribing to patients similar medications of the same pharmacotherapeutic group, but at a lower price. These drugs include:

Domestic analoguesCost, rub)Foreign analoguesCost, rub)

Gepasolone Lidocaine-Vial Lidocaine Hydrochloride Tolperisone-OBL
As for analogues with the same main components, among them we can highlight:

  • "CALMIREX". The solution is produced by the Russian JSC Pharm and is intended for intravenous and intramuscular administration. It is supplied to pharmacies in packs of 5 and 10 ampoules, and its cost starts from 310 rubles. The main advantage is the rapid action, the main disadvantage is the impressive number of possible side effects. Unlike Mydocalm-Richter, KALMIREX has not been put into circulation in Russia indefinitely. The current marketing authorization is valid until September 2021;
  • "Mydocalm." These film-coated tablets are available in dosages of 50 mg and 150 mg. All stages of production of the drug take place at the Russian JSC Gedeon Richter-RUS, due to which its price starts from 300 rubles per package. This option is perfect for those who for some reason do not want or cannot give injections. The key advantages of tablets are mobility (since no special preparation is required to take them) and more flexible storage conditions;
  • "Tolisor". These hard gelatin capsules are commercially available in 2 colors: creamy yellow (for a 50 mg dosage) and light gray (for a 150 mg dosage). The drug is produced by the Russian Ozon LLC, but it has not been put into circulation in Russia indefinitely. The current registration certificate is valid until September 2020. The price of Tolisor starts from 150 rubles, which is one of its key advantages. As for the disadvantages of the drug, among them are: a large number of side effects and the absence of a specific antidote for overdose;

Calmirex Mydocalm Tolisor
Like Mydocalm-Richter, the listed products are available in pharmacies only with a prescription. If a pharmacist is ready to sell them to you even in the absence of authorization documents, then most likely you will receive a counterfeit or a so-called “generic” that has not passed the necessary clinical tests and therefore is not included in the State Register of Medicines.

The prices given are relevant for Moscow and the Moscow region. However, they can vary both up and down (depending on: the form of release, whether the buyer has the right to a discount, etc.).

What else can you replace?

The drug Tolperisone can be found on pharmacy shelves in both tablet form and injectable form. It is rightly considered the main analogue of Mydocalm. "Tolperisone" is available in almost any modern pharmacy, and the price in comparison with the described "Mydocalm" is approximately half as low. At the same time, it is a fairly effective product containing the same main ingredient. You can use Tolperisone for muscle spasms and high tone of such fibers. The drug is often prescribed in the period after surgery to increase the effectiveness of the rehabilitation process.

Quite rarely, Baclofen is suitable as an analogue. Its main weakness is the abundance of undesirable consequences. As a rule, if a patient is prescribed Mydocalm, a replacement for Baclofen is made only in an exceptional situation. Changing one of these products to another on your own is strictly prohibited, since the course may be ineffective and even dangerous for the patient.

Similar medications

Analogues of Mydocalm are available in a wide range.

When the use of Mydocalm is contraindicated or for other reasons it cannot be used for osteochondrosis, the doctor selects an equally effective replacement. Often the pharmaceutical drug is replaced with “Miolgin” based on analgin or “Tolperisone”, intended to reduce muscle tone. A popular analogue is Sirdalud, which perfectly eliminates painful muscle spasms, as well as Baclofen, however, it is prescribed in extreme cases, as it is dangerous due to many side effects. Instead of Mydocalm, Diclofenac, which belongs to the group of non-steroidal anti-inflammatory drugs, can relieve pain. Do not forget that the selection of a similar remedy should be carried out exclusively by a specialized physician.

Methods of application, duration of treatment

How to take Mydocalm tablets, how much can you take? The dosage and duration of use directly depend on the clinical picture and severity of the pathology, the patient’s age and the presence of concomitant diseases. When choosing a treatment regimen, it is also necessary to take into account the part of the affected spine and other factors. The standard adult dosage is 50 mg. Patients are often interested in how to take pills for osteochondrosis, how to drink mydocalm. It is recommended to divide the dose into three times; in case of spasms and complex course of the disease, a threefold increase in the dose is allowed. This means that patients should take Mydocalm 3 times a day after meals, 150 mg.

The dosage for intramuscular administration is 100 mg 2 times a day, for intravenous administration - 100 mg 1 time a day. How to drink mydocalm for children, how much to take? The dosage regimen depends on the age and weight of the child:

  • 1-6 years - 5 mg per 1 kg of weight;
  • 7-14 - 2-4 mg.

If there are problems with swallowing, the tablet must be crushed into powder; the average duration of treatment with Mydocalm is 10 days. For various diseases, adjustment of the duration of therapy is allowed.

Treatment of neurological or other pathological conditions accompanied by hyperkinesis is often complex. The doctor additionally prescribes anti-inflammatory ointments (for severe pain). Therapy for some diseases may additionally include gymnastics, physiotherapeutic techniques, massage, etc. How quickly does mydocalm tablets work? The drug begins to act in approximately 40 minutes, the duration of action is 4-6 hours (depending on the functioning of the patient’s kidneys).

Therapeutic regimens

The duration of treatment in each clinical case is determined by the doctor depending on the symptoms, the general condition of the patient and the presence/absence of positive dynamics in treatment. As a rule, the minimum treatment course is 5 days.

On average, Mydocalm can be injected for up to 10 days per course. After which, if necessary, they switch to taking pills. Mydocalm injections can be administered intramuscularly or intravenously.

If injection into a muscle is prescribed, the injection can be given 1-2 times a day, 1 ampoule (1 ml), which contains 100 mg of tolperisone. The daily dose in this case should not exceed 200 mg of the main active ingredient.

If intravenous administration is intended, then the injection is given once a day. After 10 days of parenteral administration, they switch to the tablet form of Mydocalm. The maximum daily dose of tolperisone should not exceed 150-450 mg. The physician should decide how long maintenance therapy should be administered after taking a history and physical examination.

Patients with serious neurological diseases are often forced to resort to Mydocalm injections. They believe that they can inject Mydocalm once a year without thinking at all. But if you have to deal with such injections often, then the therapeutic regimens must be coordinated with your doctor.

Especially if the patient has serious problems with the kidneys and liver, then in this case, Mydocalm injections can only be taken with the agreement of several specialists - neurologists, nephrologists and hepatologists. In some cases, a dose adjustment or complete discontinuation of the drug may be necessary.

In any case, prescribing Mydocalm injections is the prerogative of a doctor, and a qualified specialist should also monitor the therapeutic process. If you give injections in accordance with the instructions, as well as in combination with drugs from other medicinal groups and physiotherapeutic procedures, then you will not need to exceed the dose or duration of treatment.

Composition and properties of Mydocalm (injections)

1 ml of solution contains:

  • active ingredients: tolperisone hydrochloride 100 mg, lidocaine hydrochloride 2.5 mg;
  • excipients: methyl parahydroxybenzoate, diethylene glycope - monoethyl ether, water for injection.

Description: Transparent, colorless or slightly greenish solution with a specific odor.

Pharmacotherapeutic group: Muscle relaxants. Centrally acting muscle relaxants. Other centrally acting muscle relaxants.

ATX code: M03BX

Pharmacological properties

Pharmacokinetics

Time of maximum concentration (Tcmax) 0.5 - 1 hour.

Bioavailability is about 20% (due to the pronounced “first pass” effect through the liver).

Tolperisone is metabolized in the liver and kidneys.

The pharmacological activity of the metabolites is unknown.

The half-life (T½) after intravenous administration is approximately 1.5 hours.

Excreted by the kidneys in the form of metabolites (99%).

Pharmacodynamics

Mydocalm is a centrally acting muscle relaxant.

The mechanism of action is not fully understood.

As a result of membrane-stabilizing and local anesthetic effects, it makes it difficult to conduct excitation in primary afferent fibers, blocking mono- and polysynaptic reflexes of the spinal cord.

The secondary mechanism of action is likely to be by blocking transmitter release by blocking the entry of calcium ions into synapses.

Reduces readiness for reticulospinal reflexes of the brain stem.

In numerous experimental animal models, the drug reduces the increased muscle tone and stiffness caused by decerebrate.

Strengthens peripheral blood circulation.

This effect is not associated with the effect of the drug on the central nervous system.

It may be due to the weak antispasmodic and antiadrenergic effects of tolperisone.

Indications for use Mydocalm

  • hypertonicity and spasm of striated muscles in organic diseases of the central nervous system (damage to the pyramidal tracts, multiple sclerosis, stroke, myelopathy, encephalomyelitis);
  • muscle hypertonicity and spasm in diseases of the musculoskeletal system (spondylosis, spondyloarthrosis, cervical and lumbar syndromes, arthrosis of large joints);
  • rehabilitation after orthopedic and traumatological operations;
  • obliterating vascular diseases (vascular atherosclerosis obliterans, diabetic angiopathy, thromboangiitis obliterans, Raynaud's disease), diseases occurring with arterial vasospasm and impaired vascular innervation (acrocyanosis, intermittent angioedema).

Application of Mydocalm (method, dose)

Adults intramuscularly or intravenously slowly.

100 mg 2 times a day, intramuscularly or 100 mg 1 time a day intravenously (once), the average duration of treatment is 7 days.

The duration of treatment is determined by the clinical picture of the disease and the attending physician.

Patients with impaired renal function

Data on use in patients with impaired renal function are limited.

A higher incidence of adverse reactions was observed in this group of patients.

Patients with twilight renal impairment should have the dose titrated and monitored closely.

The use of tolperisone is not recommended in patients with severe renal impairment.

Patients with liver dysfunction

These applications in patients with impaired liver function are limited.

A higher incidence of adverse reactions was observed in the bottom group of patients.

Patients with moderate hepatic impairment require dose titration and careful monitoring.

The use of tolperisone is not recommended in patients with severe liver dysfunction.

Drug interactions

Studies of the pharmacokinetic interaction of drugs with the CYP2D6 marker substrate dextromethorphan have shown that the simultaneous use of tolperisone can increase the blood levels of drugs that are predominantly metabolized by CYP2D6 (thioridazone, tolterodine, venlafaxine, atomoxetine, desipramine, dextromethorphan, metoprolol, nebivalol, perphenazine).

Laboratory experiments on human liver microsomes and human hepatocytes did not show significant inhibition or induction of other CYP isoenzymes (CYP2B6, CYP2C8, CYP2C9 CYP2C19, CYP1A2, CYP3A4)

Increased exposure to tolperisone is not expected with concomitant use of CYP2D6 substrates and/or other drugs due to the diversity of tolperisone's metabolic pathways.

Although tolperisone is a centrally acting drug, its sedative effect is very low.

In case of simultaneous administration with other centrally acting muscle relaxants, the dose of tolperisone should be reduced.

Tolperisone enhances the effect of niflumic acid, therefore, with simultaneous use, the possibility of reducing the dose of niflumic acid or other NSAIDs should be considered.

special instructions

Before parenteral use of Mydocalm, you should ensure that the patient does not have hypersensitivity to lidocaine.

In cases of known hypersensitivity to lidocaine, increased caution should be exercised during the use of tolperisone due to possible cross-reactions.

Female patients with hypersensitivity reactions to other drugs or a history of allergic reactions are at higher risk.

Patients should be advised to be alert for any symptoms of hypersensitivity.

Allergic reactions ranged from mild skin reactions to severe systemic reactions, including anaphylactic shock.

Symptoms of an allergic reaction are erythema, rash, urticaria, pruritus, angioedema, tachycardia, hypotension and shortness of breath.

If symptoms develop, stop tolperisone immediately and consult a doctor immediately.

Tolperisone should not be re-prescribed after an episode of hypersensitivity to tolperisone.

Pregnancy and lactation

According to preclinical studies, tolperisone does not have a teratogenic effect.

Due to the lack of clinical studies, the use of Mydocalm during pregnancy, especially in the first trimester, is possible only if the expected benefit clearly outweighs the risk of harm to the fetus.

The use of tolperisone during lactation is contraindicated due to the lack of relevant data on the penetration of tolperisone into breast milk.

Features of the effect of the drug on the ability to drive a vehicle or potentially dangerous mechanisms

During the treatment period, care must be taken when driving vehicles and engaging in other potentially hazardous activities that require increased concentration and speed of psychomotor reactions.

Technical information tablets

"Mydocalm" for oral administration is manufactured in the form of tablets coated with a thin film, which provides the active ingredient with protection when passing through the upper gastrointestinal tract. The shell is made either in white or in a shade close to it. Each specimen is round and convex on both sides. There is an engraving on one of the surfaces. If you sniff, you can feel a specific smell. If you break the tablet, you can see the white contents. A slight change in shade is allowed, but the color should be close to white.

The main ingredient indicated in the instructions for Mydocalm is tolperisone. In tablets it is used in the form of hydrochloride. One copy contains 50-150 mg of the active compound. Silicon and titanium compounds, mellose, starch, talc, acids (citric, stearic), lactose, cellulose are used as additional ingredients. The manufacturer used macrogol. The tablets are packaged in individual blisters. One cardboard box contains up to three blisters.

Treatment results

In order to effectively treat osteochondrosis, quite a lot of Mydocalm may be required. Exactly one package is enough for a course of 10 days. Tablets are available in volumes of 50 and 150 mg of active ingredient. This condition is provided for ease of use. Considering that each pack costs about 400 rubles, for monthly treatment of osteochondrosis you will have to purchase Mydocalm in the amount of 1,200 rubles. Injections are more expensive. Each Mydocalm ampoule costs about 100 rubles. Often, treatment for osteochondrosis begins with injections, and after a while they switch to tablets. How to take Mydocalm in this case and after how long to change the method of administering the medicine is determined by the doctor.

The results of treatment of osteochondrosis with the drug Mydocalm in combination with other methods and medications are usually not long in coming. With proper use and impeccable adherence to the neurologist’s recommendations, relief comes quickly. Already on the first day of taking Mydocalm, the symptoms of osteochondrosis (in particular, pain) become less noticeable. On the third day of therapy, the maximum effect is achieved. It can be used to evaluate the effectiveness of using tablets and the need to increase their dosage.

When a doctor prescribes Mydocalm, a person with osteochondrosis should learn in detail about the treatment regimen:

  • how to take Mydocalm;
  • how many times a day to drink and how many tablets to take at a time;
  • is it worth using injections instead of pills;
  • After what time can you stop taking the medicine or reduce its dosage?

Osteochondrosis of the cervical spine, which is most often diagnosed in patients, entails dangerous consequences. One of them is brain hypoxia. To prevent it, you need to take Mydocalm exactly as much as prescribed by your doctor. It happens that patients feel relief from the symptoms of osteochondrosis and stop treatment on their own. In this case, the spasm of muscle tissue persists, and the vessels continue to supply a small amount of oxygen to the brain. As a result, complications develop, and patients talk about a strange coincidence of circumstances: treatment was carried out, but it did not help.

Mode of application

Mydocalm in the form of an injection solution can be injected both intramuscularly and intravenously. The medication belongs to the aminoketone group. It is prescribed when you need to relieve muscle pain, eliminate spasticity and hypertonicity, and relax the muscles. Injections do not affect the state of the central nervous system.

Chemical composition of the solution:

  • 2.5 mg lidocaine;
  • 1 ml tolperisone hydrochloride;
  • distilled water;
  • diethylene glycol monoethyl ether;
  • methyl parahydroxybenzoate.

The drug is prescribed in injections less often than in tablets. It is used both separately and as part of complex therapy, including in combination with other medications. Common indications for use:

  • Raynaud's syndrome as a consequence of vasospasm;
  • bulbar disorder (weakness of striated muscles);
  • muscular dystonia;
  • spinal dystrophy;
  • spinal cord injuries;
  • condition after a stroke;
  • arthrosis, multiple sclerosis;
  • lumbar syndrome developed as a result of thoracic or lumbar osteochondrosis;
  • inflammation of the brain or spinal cord caused by infection, allergic reaction or intoxication;
  • spasticity and hypertonicity of muscle tissue;
  • complications of diabetes mellitus;
  • insufficient blood supply to blood vessels.

Relatively few side effects resulting from injections have been identified, but they are all very serious. If the dosage is accidentally or intentionally exceeded, the following phenomena are observed:

  • convulsions;
  • shortness of breath, up to paralysis of the respiratory organs;
  • loss of coordination due to muscle weakness;
  • disruption of the respiratory system, which can lead to cardiac arrest and death.

Mydocalm - indications for use

The drug is actively used in medicine. Doctors prescribe it for the treatment of diseases of the central nervous system and musculoskeletal system. Often these pathologies are accompanied by increased muscle tone, muscle tension, and circulatory disorders in the extremities.

The chemical structure of tolperisone is similar to lidocaine. This determines its main properties:

  • decreased electrical excitability of motor neurons;
  • blockade of primary afferent fibers;
  • inhibition of the activity of voltage-gated sodium channels.

Considering these features of the action of Mydocalm, the indications for its use are as follows:

  • osteochondrosis;
  • spondylosis;
  • lumbago;
  • spondyloarthrosis;
  • arthrosis of large joints (elbow, knee, hip).

Mydocalm tablets are also used if necessary to relieve muscle spasms caused by disruption of the nervous system and brain.

Such pathologies include:

  • stroke;
  • multiple sclerosis;
  • encephalitis;
  • myelopathy;
  • encephalopathy;
  • Little's disease.

Very rarely, qualitative changes in the blood can be observed, such as an increase in the number of leukocytes, a decrease in the number of platelets, an increase in bilirubin in the blood, an increase in creatinine concentration, and disturbances in kidney enzymes.

If the recommended dosage is observed, as well as the duration of the course of therapy, side effects develop extremely rarely. In general, the drug is tolerated very well by patients.

Cases of overdose and side effects

There is no exact data on the body's reactions during an overdose of the drug. According to studies conducted on rats, it was found that in cases of an increase in the concentration of the active substance in the blood above the norm, the following are observed:

  • some disorders of coordination of movements, which is provoked by muscle weakness;
  • difficulty in the respiratory functions of the body, possibly leading to complete cardiac arrest and death.

If the dosage is exceeded, gastric lavage followed by maintenance therapy is recommended.

In general, the following side effects are observed when Mydocalm is abused:

  • increased irritability;
  • convulsions;
  • shortness of breath, contributing to respiratory paralysis.

Drug kinetics

The drug in question is well absorbed from the intestines. Its concentration (maximum) in the blood is reached 65 minutes after administration. Metabolism of the drug occurs in the kidneys and liver. Tolperisone derivatives are excreted in the urine.

A repeated course of Mydocalm does not have any effect on the functioning of the kidneys and liver, as well as on the process of hematopoiesis.

This remedy reduces experimentally induced rigidity and hypertonicity of muscle tissue. The medicinal properties of this medicine are actively used to reduce muscle rigidity and pathological hypertonicity, which are associated with extrapyramidal disorders.

It should also be noted that this drug promotes human activity (his movements). In addition, it does not affect the cerebral cortex in any way, fully maintaining the patient’s level of wakefulness.

According to the instructions, treatment with Mydocalm causes slightly pronounced antispasmodic and adrenergic blocking effects. Thanks to this, tissue perfusion improves.

During long-term clinical studies, this drug did not have a pronounced effect on hematopoiesis and kidney function.

Pharmacological profile

A drug that helps reduce the tone of the skeletal system, which is associated with a decrease in motor functions, the worst option of which is a complete restriction of movements.
The mechanism of action of the drug is aimed at normalizing the transmission of information signals from the brain to the upper spinal cord, while reducing the level of reflex excitability of the latter.

Promotes the transmission of intracellular signals in skeletal muscle tissue, is an H-cholinergic. Tolperisone contained in the drug reduces muscle spasms and reduces the impact of diseases that are accompanied by increased tone or dystonia.

The secondary effect of the drug is a decrease in the transmission of nerve impulses during the intake of calcium saturated with ions into the body. Normalizes the stability of motor functions. Lidocaine contained in the drug has local anesthetic properties.

Chemical reactions that occur during exposure to the drug are metabolized at high speed in organs of the digestive system such as the liver and kidneys. Mydocalm is excreted in the urine by more than 99% within about an hour and a half.

Mechanism of action

This direction has not been fully studied, but the main function of the drug is aimed at correcting the transmission of impulses from the brain to the spinal cord, as well as reducing the increased excitability of the latter. Regulates the transmission of signals between nerve cells throughout the body.

Causes a slowdown in the action of biologically active substances for the transmission of nerve impulses or the flow of calcium ions to the points of contact between neurons.

What patients say about the drug

Reviews from patients diagnosed with osteochondrosis who have taken or are taking Mydocalm prove the effectiveness and safety of the drug.

I met Mydocalm two years ago. It was recommended to me by my attending physician. At that time, I was simply writhing in pain, I could neither sit nor stand. A few injections literally brought me back to life. And - most importantly - the effect lasts for quite a long time. Now, as soon as osteochondrosis makes itself felt, I take Mydocalm tablets. Andrey, St. Petersburg.

Mydocalm helped my husband a lot. The last time he had an acute attack of lumbar osteochondrosis, neither Movalis nor Nimesil helped. Ointments also turned out to be useless. Then the doctor recommended a course of treatment with Mydocalm. It cannot be said that my husband’s back does not hurt at all after this, but there have been no acute attacks for a long time. Elena, Ekaterinburg.

Therapeutic effect

Mydocalm injections can have an analgesic and relaxing effect on the human body, helping to increase and facilitate motor ability. This drug does not affect the patient's consciousness or alertness. That is, Mydocalm injections do not cause psychomotor inhibition or other similar conditions in people.

In general, these injections can provide a local anesthetic and membrane-stabilizing effect, which improves lymphatic and blood circulation, achieves a vasodilatory effect, and at the same time reduces muscle spasms. Thanks to the presented drug, muscle hypertonicity decreases in patients along with rigidity due to osteochondrosis and motor activity is facilitated.

The main active ingredient of the drug is tolperisone. This ingredient is highly related to human nerve tissue. Its effect is aimed at reducing the sensitivity of nerve endings, and in addition, at relaxing muscles, inhibiting spinal reflex centers and normalizing peripheral circulation. The substance tolperisone helps lower the pain threshold, blocking some reflexes and reducing hypertonicity of the muscles and muscles.

Despite the fact that drugs with a similar mechanism of action when taken for a long time cause many adverse reactions, the pharmacological effect of Mydocalm injections is carried out without pathological effects on the hematopoietic system or kidneys.

The presented drug is well tolerated, including it is well accepted by elderly patients, as it does not cause any cardiotoxic or sedative effect. In addition, this medicine does not provoke cognitive impairment in the form of memory impairment, decreased mental performance, and the like.

Pharmacological properties

Mydocalm is a central muscle relaxant. Its pharmacological action is due to the main active component - tolperisone hydrochloride. It has an inhibitory effect on spinal reflex centers.

This muscle relaxant component has the following effects on the body:

  1. reducing spasticity of muscle fibers;
  2. reduction of pathological muscle hypertonicity;
  3. decreased muscle resistance when trying to perform passive movements;
  4. reducing the intensity of pain in skeletal muscle tissue;
  5. facilitating voluntary active movements and increasing their amplitude;
  6. improvement of peripheral blood circulation;
  7. slight vasodilator effect.

Mydocalm is a product of the Hungarian pharmaceutical company Gedeon Richter. The drug is available in the form of an injection solution and also in the form of tablets.

Mydocalm has proven itself well in neurological practice and compares favorably with other analogues. Patients, as a rule, leave only positive reviews about these injections.

Analogs

Generics (analogues in composition) are drugs such as Tolperil, Midostad Combi, Miaxil.

“Mydocalm” differs from these medications only in price and the company that produces it. There are no differences in safety and pharmacological properties.

Analogues that differ in composition but have a similar therapeutic effect include: “Tizanidine”, “Tizalud”, “Sirdalud”, “Muskomed”, “Myoflex”, “Miorix”, “Baclofen”.

Each of these drugs can be replaced with Mydocalm, since they are all its analogues and synonyms. However, the replacement of the medicine must be carried out by a doctor.

Detailed instructions for Mydocalm analogues are included in each pack of the drug.

Features of the composition

The drug is produced by a pharmaceutical company from Hungary. Available in liquid form. Contains 2 main components - tolperisone hydrochloride and lidocaine.

The composition contains excipients in the form of:

  • methyl parahydroxybenzoate;
  • monoethyl ether;
  • water for injection.

The solution is in ampoules, the volume of which is 1 ml. There are 5 pcs in 1 cardboard box. and an application note.

Overdose

Information about an overdose of the drug has not been recorded; mydocalm has a wide spectrum of action and is non-toxic. There are described cases of oral administration of 600 mg of the main active ingredient in children; there were no toxic manifestations. When children took 300-600 mg of the drug, increased irritability was observed. There is no specific antidote; if an overdose is diagnosed, gastric lavage is performed; at the next stage, symptomatic/supportive therapy is recommended.

Mydocalm and transport management

It must be remembered that the medication can cause drowsiness and dizziness , therefore, in order to avoid dangerous consequences while driving, it is not recommended to take Mydocalm immediately before driving. In addition, patients experience blurred vision and impaired attention during Mydocalm therapy.

Main analogues of the drug

The modern pharmaceutical market is famous for its wide range of medicines. For almost every drug, you can choose an analogue that is similar in its mechanism of action. “Mydocalm” was no exception – a mega-popular medicine, the analogues of which there are many:

  • "Sirdalud";
  • "Tolperisone";
  • "Tolperil";
  • "Mefidol";
  • "Lioresal";
  • "Miolgin";
  • "Baclofen."

Before replacing Mydocalm-Richter (the instructions warn about this), you need to consult with your doctor, since only he can make a qualified selection of a particular medication.

"Surdalud"

One of the most popular analogues of Mydocalm. This is a potent remedy that can be taken once a day; it effectively relieves even the most severe pain. When taking the drug, be aware that it may cause excessive fatigue and drowsiness.

"Tolperizon"

The presented drug is a centrally acting muscle relaxant. Often prescribed for the treatment of muscle hypertonicity and obliterating vascular diseases. This product is approximately twice as expensive as the original.

"Miolgin"

The analgesic and antipyretic effect of the drug is due to the presence of paracetamol in its composition. "Miolgin" is prescribed for neuralgia, sprains and muscle strains. This is a fairly effective drug for headaches. Most experts do not recommend taking this medicine to patients under 18 years of age, nursing mothers, or pregnant women.

"Baclofen"

This remedy is prescribed in extreme cases, as it has a number of side effects.

It is important to remember that when choosing a particular analogue of the drug, just reading the instructions for use is not enough. You need to choose a medication very carefully, strictly following the instructions of your doctor.

Analogues of the drug for injections Mydocalm Richter

In pharmacology, all drugs are usually divided into synonyms and analogues:

  1. Synonyms are drugs that contain one or more identical active ingredients designed to have a therapeutic effect on the body.
  2. Analogs are medicines that may contain completely different active ingredients.

But most often, if you ask for an analogue of a drug at a pharmacy, the pharmacists will offer exactly what has a similar composition (synonym), but can cost much less. Somehow these concepts got mixed up. And although, sometimes, generics are not at all inferior to the original drug, it is still worth not forgetting that Gedeon Richter is a quality therapy across generations.

Synonyms of Mydocalm Richter injections:

  • Tolperisone;
  • Tolperil;
  • Midostad Combi.

Such drugs have similar dosages and are easily interchangeable. Some patients boldly allow themselves to find an alternative on their own.

Here are the analogues of the drug:

  • Lioresal Intrathecal;
  • Mefedol;
  • Baclofen;
  • Sirdalud;
  • Tizalud.

These are also muscle relaxants, but their effects on the body may differ from the effects of Mydocalm, so you should not experiment on your own. Analogues must be prescribed by a doctor!

Instructions and possible contraindications of Mydocalm

My doctor said that antispasmodics can be harmful to health if overdosed. Therefore, it is very important to carefully read the manual for the drug, and it is better to consult a specialist. According to the instructions, for osteochondrosis you need to take from 150 to 450 mg per day. I started with 3 tablets a day. I took them immediately after meals, without chewing. You can drink it with plain, warm, boiled water (half a glass). Unfortunately, in the second week I had to increase the dosage slightly. The effect of Mydocalm is felt within 30-40 minutes after administration.

The description of the drug also indicated side effects. You need to carefully monitor your feelings. If you experience a headache, nausea, vomiting, itchy skin, or a drop in blood pressure, then you need to stop taking the pills and consult a doctor to select suitable analogues.

I also read that Mydocalm is prohibited:

  • people suffering from intolerance to any of the above-described components of the drug;
  • small children (up to one year);
  • patients with muscle tissue problems (myasthenia gravis).

Particular caution should be taken when taking the drug if you have epilepsy, renal failure, or drug addiction. I don’t suffer from anything like that, so I drank Mydocalm quite calmly.

pharmachologic effect

Mydocalm is a highly effective and safe remedy for the treatment of neurological syndromes that are associated with pain, increased muscle tone and impaired trophism. The active ingredient Mydocalm has a membrane-stabilizing and local anesthetic effect. The drug reduces musculoskeletal tension and causes an indirect analgesic effect, as well as dilates blood vessels and improves blood and lymph circulation. According to reviews, Mydocalm has a rapid effect and is quite well tolerated.

Negative reviews

There are few negative reviews from patients who took Mydocalm. But they are not associated with the lack of therapeutic effect of the drug.

Patients note the following:

  • effectiveness only during treatment; after discontinuation, cramps and numbness return;
  • bitter taste - when prescribing the drug to children, the tablets have to be divided. The taste cannot be masked by adding juice, milk, or jam;
  • 1% of patients - the drug was completely ineffective;
  • relaxation of smooth muscles - this was expressed in impaired defecation, pain in the intestines, frequent urination; in women there was pain in the pelvis;
  • 1% of patients developed allergic reactions - accompanied by itching, fever, and rash. In severe cases, bronchospasm, asthma attacks, and Quincke's edema were present.

Indications for Mydocalm injections

Used for damage to the musculoskeletal system:

  • Osteochondrosis of various parts of the spine - used to reduce the intensity of pain and relax the paravertebral muscles;
  • Pinched nerve – radiculitis;
  • Spinal cord pathology;
  • Residual period of hemorrhagic and ischemic strokes;
  • Brain lesions that cause muscle dysfunction - Little's disease, cerebral palsy;
  • Progressive diseases of the nervous system;
  • Pathological increase in muscle tone - as a result of amputations of limbs, operations with damage to nerve trunks and endings;
  • Inflammatory and non-inflammatory joint disorders.

Indications from the circulatory system:

  • Disturbances of blood flow in the brain and spinal cord - traumatic, inflammatory in nature;
  • Restoring blood flow in the event of blockage of veins, arteries, and lymphatic vessels by blood clots;
  • Progressive sclerosing diseases – obliterating endarteritis;
  • Damage to the vascular wall by immune complexes in autoimmune diseases - dermatomyositis, systemic lupus erythematosus, Raynaud's syndrome;
  • Early postoperative period in traumatology and orthopedics;
  • Difficulty in microcirculation in diabetes mellitus.

In urology:

  • Acute, protracted and chronic types of prostatitis;
  • To relieve the symptoms of renal colic.

In gynecology and obstetrics:

  • Used to relieve symptoms of painful periods;
  • Recommended as an antispasmodic when there is a threat of spontaneous miscarriage in the first 12 weeks of pregnancy (relieves uterine tone).

In gastroenterology:

It is used as an antispasmodic and analgesic when other drugs are ineffective. For example, during exacerbation of cholelithiasis.

Drugs with a similar effect

Analogues containing the same active ingredient as Mydocalm:

Among these remedies you can find cheaper drugs than Mydocalm. But effectiveness may vary. Mydocalm has been the most studied, and there is extensive experience in using it in clinical practice. It is recommended that you consult your doctor before purchasing or changing medications.

Analogue drugs containing a different active substance, but characterized by a similar effect on the body, prescribed for the same conditions:

The estimated cost of such drugs ranges from 200 to 380 rubles. The cheapest from this list is Sirdalurd. Its price ranges from 220 to 300 rubles.

Pharmacological belonging to the same group of Mydocalm and Sirdalurd gives rise to possible interchangeability. However, they contain completely different substances. Mydocalm appeared on the pharmaceutical market earlier, and therefore is used more widely. The main difference between the drugs is the less frequent occurrence of adverse reactions when taking Sirdalurd. However, it much more often causes such unpleasant symptoms as drowsiness and severe apathy. During treatment, driving a car or performing work in which speed of response is important is not recommended.

Pharmacodynamics

The exact mechanism of action of the drug “Mydocalm-Richter” (injections) on the human body has not yet been deciphered. Scientists suggest that due to the effect of the drug on afferent fibers, poly- and monosynaptic reflexes of the spinal cord are blocked. The bioactive components of Mydocalm-Richter inhibit the release of the transmitter by blocking the entry of Ca2+ ions into synapses. Tolperisone hydrochloride exhibits a weak antispasmodic and adrenergic blocking effect. And that’s not all for the drug “Mydocalm-Richter” indications for use. It activates peripheral blood circulation and improves the metabolism of substances in the pathological area.

Compound

Before talking about what Mydocalm tablets help with, it is necessary to consider their composition. The international nonproprietary name of the drug is Tolperisone. Its active component is tolperisone hydrochloride in amounts of 50 mg and 150 mg. The tablets also contain excipients:

  • talc;
  • corn starch;
  • lactose monohydrate;
  • colloidal silicon dioxide, etc.

IMPORTANT! Details about the components included in the drug are described in the annotation. Be sure to study it before starting treatment, since if you have an intolerance to at least one substance, taking the medicine is contraindicated.

Pharmacokinetic properties

What Mydocalm-Richter injections help with should never be the only criterion when prescribing treatment. The patient’s metabolism and the pharmacokinetic properties of the prescribed drug also play an important role in this matter. So:

  1. Tolperisone hydrochloride is processed primarily by the liver and kidneys. After intravenous administration, it is excreted in the form of metabolites approximately 1.5 hours after the injection;
  2. Lidocaine hydrochloride is completely absorbed by the body. The speed depends on the injection site and dosage. For intravenous injections it takes about 40-45 minutes. The substance is able to penetrate the placental barrier and be secreted into breast milk (percentage ratio: 40% to 60%). It is excreted mainly with bile, up to 10% is excreted by the kidneys.

Due to the described pharmacokinetic properties, doctors strongly do not recommend Mydocalm-Richter to nursing mothers and pregnant girls (especially those who are no more than 12-15 weeks pregnant).

The effect of a muscle relaxant on a child’s body has not been properly studied, and therefore it can pose a threat to the baby’s health.

Drug overdose

The range from the minimum to maximum daily dose of the drug is quite large. If you take the medicine in excess of the norm, serious complications, as a rule, do not occur. According to studies, taking 600 mg of Mydocalm in children did not provoke any negative consequences.

Despite this, experts still recommend performing gastric lavage if you take too large a dose of Mydocalm. If you administer too large a dose of Mydocalm as an injection, an allergic reaction in the form of a small rash may appear on the skin.

Composition, release form, storage conditions

To understand what Mydocalm-Richter helps with, just look at its composition. Thus, the main components of the solution are tolperisone and lidocaine hydrochlorides. 1 ml of the drug contains 100 mg and 2.5 mg, respectively.

The following additional elements are used:

  • Distilled water;
  • Methyl parahydroxybenzoate;
  • Diethylene glycol monoethyl ether.

The solution is intended for intravenous and intramuscular administration. Presented as a colorless or slightly greenish liquid, it is endowed with a specific, rather pungent odor.

The drug is produced in Hungary by the open joint-stock company Gedeon Richter. In Russia, the joint stock company GEDEON RICHTER-RUS is responsible for secondary packaging and release quality control.

The drug is supplied to pharmacies in cardboard packages, each of which contains instructions for using Mydocalm-Richter and 5 ampoules (1 ml capacity) made of dark glass with a breaking point.

The solution is available only by prescription and must be stored in accordance with the requirements for drugs from list “B”, namely: in a dry, dark place at a temperature of 8-15 °C. Shelf life – 3 years. Since the drug is not intended for children, their access to it should be limited.

"Mydocalm-Richter" is a potent medicine and is sold only with a prescription. Therefore, if on the Internet or somewhere else they agree to sell a drug without it, then most likely they are trying to sell you a counterfeit.

Reviews

Mydocalm is attributed from time to time to my father-in-law. Five years ago he was diagnosed with multiple sclerosis. After 5 years, my arms and legs move poorly. He undergoes treatment three times a year. Each time it is attributed to Mydocalm. He says that after taking it, pain in the limbs and back decreases and mobility returns a little. It is very good that he has no contraindications, which with this difficult illness would only worsen his father-in-law’s condition. Took 150 mg every 8 hours. Although the drug is not cheap, it fully pays for its price.

They used Mydocalm when my husband pulled a muscle in his chest. The pain was such that he could hardly do anything with his right hand; local anesthetic ointments did not help or helped for a very short time, and the symptoms did not go away. After going to the doctor, he was prescribed Mydocalm. My husband took the pills for 5 days. During this period, chest spasms decreased and the pain became significantly less. They were already treated with Voltaren. After treatment, the muscle still made itself felt for a month, but then the discomfort gradually disappeared.

I have already known Mydocalm for three years.
I use it as soon as osteochondrosis worsens. The doctor sometimes prescribes it in the form of injections, and sometimes in the form of tablets. The form mainly depends on the severity of pain syndromes. Of course, it’s more pleasant to take pills, since I don’t like injections. The spasm in the lower back is relieved within an hour, naturally, and the pain goes away as soon as the muscles relax. So, I recommend Mydocalm as a good muscle relaxant. But with high doses, such as 150 mg, I feel lethargic throughout my body. comments powered by HyperComments

Dosage and procedure for working with the ampoule

Unlike the tablet form of this drug, which comes in two dosages (50 and 150 mg), the injection form has one standard dosage. If we focus on the presence of 100 mg of tolperisone in the composition, then in the recipes it looks like this: Mydocalm 0.1 g/ml 1 ml amp. No. 5 or, more precisely, 100 mg + 2.5 mg/ml.

It is worth noting that ampoules must be stored correctly. They are suitable for temperatures ranging from 8 to 15 °C, which means they belong in the refrigerator! Accordingly, before parenteral administration, the ampoule must be warmed. To do this, you should hold it in your fist for some time. The ampoule is very convenient; it has a marked break point, so it is very easy to open.

The drug can be administered intravenously, 1 ml once a day . This needs to be done very slowly. Or intramuscularly 1 ml 2 times a day. The injection should be given in the middle of the upper outer quadrant of the buttock. A 2 ml syringe is sufficient for the procedure. The first administrations must take place under the supervision of a specialist to avoid possible allergic reactions. The duration of treatment is also determined by the doctor.

Maximum duration of use

Despite the fact that the course of treatment with Mydocalm is on average 2 weeks, some cases require long-term use. For osteochondrosis and persistent back pain, the medicine can be taken for a long time. Numerous studies have shown that Mydocalm combines well with other pharmacy products.

Medical practice shows that some patients were helped by the medication for a year, after which Mydocalm was replaced with an analogue or completely discontinued. The maximum period during which treatment with a muscle relaxant is considered is 7 years.

It is quite difficult to say unequivocally what is more effective, injections or tablets.

Some patients benefit better from tablets, but they experience various side reactions to injections of the same name. For others, on the contrary, Mydocalm injections are much more effective than its tablet form.

In general, the injection form of the drug penetrates the bloodstream much faster and begins to act than tablets, which means that the injections are more effective. In any case, only the doctor should decide which form the patient should take.

Mydocalm tablets have a fairly wide range of applications:

  • Brain pathologies of non-inflammatory etiology, which are characterized by muscle tone disorders (cerebral paralysis in children, etc.);
  • For diseases of the muscular and skeletal system such as osteochondrosis, arthrosis, sciatica, lumbago, spondylosis and other pathologies that cause muscle spasticity and hypertonicity or limitation of joint mobility;
  • For renal colic, cholelithiasis;
  • Muscle spasms and muscle hypertonicity resulting from nervous system lesions such as stroke, encephalitis, multiple sclerosis, etc.;
  • Increased uterine tone, threatening spontaneous miscarriage;
  • Severe pain in hemorrhoids;
  • Dysmenorrhea (excessively painful menstruation).

In addition, Mydocalm tablets are often included in postoperative rehabilitation therapy due to surgery, in the treatment of vascular lesions such as thromboangiitis (systemic venous or arterial inflammation), Raynaud's syndrome (circulatory disorders in the vascular network of the extremities), etc.

The drug is actively used in the treatment of pathological cyanosis of the extremities, severe gait disorders, and in post-stroke symptomatic therapy to eliminate muscle spasms.

According to the instructions, Mydocalm is used for:

  • Diseases of the musculoskeletal system - spondyloarthrosis, spondylosis, cervical and lumbar syndromes, arthrosis of large joints, which cause muscle spasms, pathologically increased tone and muscle contractures;
  • Spastic cerebral palsy and other encephalopathies accompanied by muscular dystonia;
  • Pathologically increased tone and spasms of striated muscles, which arose as a result of organic neurological diseases - multiple sclerosis, myelopathy, damage to the pyramidal tracts, stroke or encephalomyelitis;
  • Rehabilitation treatment after traumatological and orthopedic operations.

Mydocalm is also used as part of combination therapy in the treatment of:

  • Obliterating arteriosclerosis;
  • Intermittent angioedema;
  • Raynaud's disease;
  • Diabetic angiopathy;
  • Acrocyanosis;
  • Thromboangiitis obliterans.

The dose of Mydocalm according to the instructions is prescribed individually depending on the tolerability of the drug and the clinical picture.

Mydocalm tablets are taken after meals. Typically, adults take 1 tablet (50 mg) up to three times a day. If necessary, the dose can be tripled by taking 3 Mydocalm tablets three times a day.

For children, Mydocalm is allowed to be used in the form of 50 mg tablets. The daily dose of Mydocalm is taken in three doses:

  • 1-6 years - calculating the daily dose, 5 mg per 1 kg of weight;
  • 7-14 years – 2-4 mg per 1 kg of weight.

If it is impossible to swallow a Mydocalm tablet, it can be crushed.

When using Mydocalm injections, the following is administered:

  • Intramuscularly - 200 mg per day, divided into two doses;
  • The drug is administered intravenously slowly, once a day, 100 mg.

There is no data limiting the use of Mydocalm with other medications. The drug can be used simultaneously with sedatives, hypnotics and drugs containing ethanol.

However, it should be borne in mind that Mydocalm enhances the effect of niflumic acid.

Mydocalm can be used during pregnancy, especially in the first trimester, as well as during lactation only according to strict indications.

Due to the lack of data on use during pregnancy and breastfeeding, the use of Mydocalm ® during these periods is not recommended.

Inside, after eating, without chewing, with a small amount of water.

Adults and children over 14 years of age - usually starting with 50 mg 2-3 times a day, gradually increasing the dose to 150 mg 2-3 times a day.

For children from 3 to 6 years old, Mydocalm ® is prescribed orally at a daily dose of 5 mg/kg; 7–14 years – 2–4 mg/kg (in 3 doses during the day).

The medicine is intended for oral administration. It is optimal to take it immediately after a meal, without chewing and with a small amount of liquid.

Adults are recommended to start treatment with the drug with a dose of 100-150 mg per day, divided into 2 or 3 doses. Further treatment involves a gradual increase in a single dose to 150 mg. The frequency of doses remains unchanged (2-3 per day).

Typically, the dosage is selected depending on muscle tone, pain intensity, the presence of contraindications in the patient and concomitant chronic diseases.

Based on this data, the doctor also determines how many days to take the pills. The duration of the therapeutic course usually does not exceed ten days.

After oral administration, the drug begins to act after approximately forty minutes. The duration of action varies from 4 to 6 hours (it depends on how well the renal blood filtration system is functioning).

Treatment of certain diseases is supplemented with a complex of physiotherapeutic measures, gymnastics, massage courses, rubbing ointments, etc.

Description of the drug

Today Mydocalm is produced in the form of ampoules and tablets. One package usually consists of 5 ampoules or 30 tablets.

Doctors prescribe Mydocalm to increase muscle tone and activate movements in a compressed organ. The medicine has an effect on the nerve endings in the spinal cord and perfectly eliminates spasms and tightness due to bruises, injuries and osteochondrosis. Mydocalm can be bought at a pharmacy without a doctor’s prescription, but before using this drug it is recommended to consult a specialist to determine the exact dosage of the medication.

Pharmacokinetics

Metabolism of tolperisone hydrochloride occurs in the kidneys and liver. About 99% of the substance is excreted from the body in the form of metabolites of uncertain pharmacological activity. The half-life is 1.5 hours.

Lidocaine hydrochloride undergoes complete adsorption. The rate of absorption of this component depends on the dose and site of administration of the drug. When administered intravenously, the drug reaches its maximum concentration in the blood plasma within minutes. The connection with plasma proteins is 50-80%.

Mydocalm Richter is very quickly distributed throughout all organs and tissues of the human body, is able to penetrate the blood-brain and placental barrier, about 40% of the total amount of the drug contained in maternal blood plasma is secreted along with milk. The muscle relaxant is metabolized in the liver (about 90-95%). The cleavage reaction involves microsomal enzymes that dealkylate amino groups and promote the cleavage of the amide bond and the formation of active metabolites. The drug is excreted along with bile, partially reabsorbed in the intestine. About 10% is excreted unchanged in the urine.

Interaction with other drugs and other types of interactions

As a result of pharmacokinetic studies, it was revealed that when taken simultaneously, tolperisone increases the concentration in the blood plasma of the following drugs:

  • Thioridazine;
  • Tolterodine;
  • Venlafaxine;
  • Atomexitin;
  • Desipramine;
  • Dextromethorphan;
  • Metoprolol;
  • Nebivolol;
  • Perphenazine.

At the same time, general anesthesia, clonidine, psychotropics and peripherally acting muscle relaxants can enhance the effect of tolperisone.

Also, the drug does not enhance the effect of alcohol on the central nervous system , so simultaneous use of drugs based on ethyl alcohol with tolperisone is allowed.

Taking tolperisone on an empty stomach reduces its bioavailability, therefore parenteral administration is recommended after meals. Despite the general group of muscle relaxants to which tolperisone belongs, its sedative properties are weakly expressed. If you take it with other muscle relaxants, it would be wise to reduce the dosage.

Another feature of this drug is the potentiation of the effects of NSAIDs, so they should be taken in lower dosages.

There are no exact research results regarding the compatibility of Mydocalm in injections with other injectable drugs. This means mixing it in the same syringe with other medications would be wrong.
