Isofra - indications for use
The nasal spray is a prescription and is prescribed by your doctor (otolaryngologist or internist) for treatment against bacteria. The patient feels better the next day when Isofra is prescribed. The drug is used with caution (read the instructions section), because it contains an active antimicrobial substance - framycetin sulfate. This antibiotic belongs to the pharmacological group of aminoglycosides, and has repeatedly proven its effectiveness against bacteria.
Features and instructions for use of Isofra:
- the composition of 1 ml of nasal spray contains Framycetin sulfate and additional substances;
- Shelf life – standard, 3 years;
- storage at a temperature no hotter than 25 °C;
- Available in the form of a 15 ml bottle;
- There is no drop of Isofra, only a spray;
- The medication is recommended for children over 1 year of age;
- low price.
For children
A product with a pronounced antimicrobial effect is suitable for children's bodies. In ENT practice, Isofra spray is often prescribed to children. It is not recommended to select an antimicrobial spray for children on your own. Their nasal mucosa is less protected and very sensitive. When visiting a clinic, a pediatric otolaryngologist will be able to prescribe a safe duration of treatment with a bactericidal spray. The following instructions will help you find out when the antibiotic Isofra is prescribed to your child:
- with unfavorable processes in the nasal mucosa and paranasal sinuses;
- if there is inflammation in the pharynx - pharyngitis;
- as a preventative measure after surgery.
Dr. Komarovsky opposes the use of antibiotics as a treatment for runny nose in children. Only your pediatrician can tell whether this drug or regular nasal drops are suitable for your child, having determined the nature of the runny nose - bacterial or viral.

For adults
Isofra is actively used by adults instead of drops to treat advanced runny nose. It is not recommended to use the spray for normal clear discharge. The instructions say that Isofra is not ordinary drops, but a medicine with antibacterial properties. Antibiotics destroy both bad bacteria and good ones, opening access to the development of dysbiosis. Isofra for adults should be used strictly for ten days and abstain from alcohol. Framycetin (Isofra substance) is suitable for topical use and neutralizes types of bacteria such as:
- staphylococci;
- streptococci;
- gram-positive and gram-negative bacteria (pseudomonas aeruginosa, shigella);
- enterobacteria;
- facultative anaerobes (Salmonella).
During pregnancy
Isofra is prescribed during pregnancy when the body's protective functions are weakened. When immunity decreases, a runny nose turns into sinusitis. The use of an antibacterial agent during pregnancy is justified if the harm from sinusitis is greater than from the antibiotic. The instructions will help you determine whether Isofra is suitable for you, but only a doctor can assess the risk. Self-prescribing a drug containing an antibiotic during pregnancy is strictly prohibited. No studies have been conducted on the effect of the drug on the pregnant body and the lactation period.
Contraindications and possible side effects
Analysis of research results demonstrates that Isofra is quite safe and does not have many restrictions.
Manufacturers prohibit the use of the medicine in the following cases:
- in case of individual intolerance to components;
- in the presence of an allergic reaction;
- during pregnancy and breastfeeding;
- as a nasal rinse.
Remember that Isofra should not be used with antifungal medications, because they can neutralize the effect of the spray. Please note the following restrictions:
- high sensitivity of the child’s body to the active substance or additional components;
- child is less than 12 months old.
In general, the drug is well tolerated, although there have been cases of allergies. In case of an allergic reaction, use should be stopped, the nose should be rinsed carefully, and the throat should be rinsed thoroughly. If the doctor considers it necessary, take antihistamines.
With long-term treatment, the drug dries out the nasal mucosa, changing its microflora. Long-term use is addictive. If you have any negative reaction to treatment, you must inform your doctor.
Isofra - the effect of the drug
The anti-inflammatory antibiotic framycetin sulfate is injected through the opening of the spray. How does Isofra work after contact with the mucous membrane? Affects the inflamed area, destroying bacteria. Based on the instructions, the use of the drug helps in the case of treatment:
- infectious diseases of the upper respiratory tract;
- inflammation of the nasal mucosa;
- nasopharyngitis;
- sinusitis (only when there is no damage to the septum);
- inflammation after surgery.
Indications, restrictions for use
The drug is prescribed for diseases of the nasal cavity, against the background of acute respiratory viral infections and its complications, accompanied by thick green discharge, fever, and impaired nasal breathing. Indications are only pathologies of bacterial origin:
- runny nose;
- sinusitis;
- sinusitis;
- nasopharyngitis.

The medication is used as a prophylactic agent to prevent infection and inflammation after surgery on the nasal cavities. Contraindications include age under 12 months, intolerance to the aminoglycoside group or components of Isofra.
Isofra - instructions for use for children
Framycetin is a broad-spectrum drug that can fight gram-negative and gram-positive bacteria. Due to its high effectiveness and aggressive principle of action, before use, Isofra’s instructions should be carefully studied by an adult. Upper respiratory tract infections require complex treatment, and the spray is prescribed together with another drug. Before using Isofra for children, you should study several rules presented in the instructions.
- The daily consumption dose is lower than for adults: 1 dose of Isofra spray 3 times a day (morning, afternoon and evening).
- It is important to maintain an equal interval between doses.
- At the first manifestations of dysbiosis, you need to immediately inform your doctor about it, and not sit on forums and read reviews.

Up to a year
According to the results of a study on the topic “Use of framycetin Isofra for children under one year of age with acute rhinitis and acute otitis media (AOM)”, conducted in Moscow in 2003, it was revealed that Isofra has pronounced effectiveness and excellent tolerability in the treatment of acute rhinitis and otitis media (ear inflammation) in infants. The convenient form of release of the drug in the form of a spray allows the active substance (framycetin) to spread safely throughout the nasal mucosa, and accessible instructions help to use Isofra correctly.
For adenoids
Adenoiditis is a dangerous inflammation with purulent manifestations from the nose. Increased temperature, swelling and nasal congestion are possible. As prescribed by the doctor and after studying the instructions, a child with adenoids is prescribed 1 injection of an antibiotic spray 3 times a day. The drug Isofra for adenoids has shown itself to be a safe remedy for this disease, but only if the diagnosis is correct.
Composition and release form
Framycetin sulfate is an active component of the drug that has an antimicrobial effect. Excipients include methylparaben, sodium citrate, water, sodium chloride and citric acid monohydrate. Additional components stabilize the dosage form and facilitate the absorption of the active component into the tissue.
Package
Isofra is available in the form of a spray for topical use. Per 100 milliliters of liquid there are 1.25 g of active substance.
Isofra - instructions for use for adults
The medicine can be treated in the absence of contraindications and allergies. How to use the spray correctly is indicated by the instructions for use of Isofra for adults:
- Before starting the procedure, you need to rinse your nose, clearing it of mucus.
- Next, you need to spray the spray several times to the side. This is necessary to correctly determine the dosage.
- Tilt your head forward and spray one spray into both nasal passages, holding the spray vertically.
- The drug can be used up to 6 times a day.
- The duration of treatment is up to 10 days, but if improvement does not occur within a week, the drug should be discontinued.
Mode of application
The instructions recommend the use of the drug for the treatment of children over one year of age. .
However, as a result of recent studies, the safety and good tolerability of the drug in infants older than one month have been proven. Therefore, in practice, pediatricians prescribe Isofra even to infants.
The spray is suitable for babies older than one month.
When using the medicine, follow these rules:
- Before the procedure, ask your child to blow his nose or clean his nose with a cotton swab and saline solution.
- Ask your baby to tilt his head forward a little or do it yourself. If he is not yet three months old and does not hold his head well, treat his nose by laying him on a flat surface on his back and slightly turning his head to the side so that the medicine does not flow into the larynx.
- Hold the spray bottle vertically.
- Insert the tip of the spray into the nasal passage and lightly squeeze the walls of the bottle - the medicine will be evenly distributed on the inflamed mucous membrane.
- Repeat the same with the other nostril.
Remember that you cannot rinse your nose with this drug.
Pediatricians recommend repeating the procedure of 1 injection into each nasal passage 3 times a day.
The course of treatment with Isofra for children is 10 days. Usually during this time the infection completely recedes. Improvements are noticeable within 4-5 days. Continuing treatment longer than the specified period, as well as quitting it without completing the course, is not recommended. In the first case, disturbances of the normal microflora (dysbacteriosis of the nasopharynx) are possible; in the second case, the infection may re-develop.
Note! If after a week of regular use there is no improvement, contact your pediatrician again: perhaps the infection in the nasopharynx is caused by bacteria that are insensitive to Isofra spray. In this case, the doctor will prescribe another treatment.
Isofra is practically not used for the prevention of rhinitis. In exceptional cases, a specialist may prescribe a course of treatment with the drug after surgery on the nasopharynx to prevent a hospital infection - any disease of microbial origin that the child contracted within the walls of the hospital. As a rule, it is severe and cannot be treated due to the resistance of “hospital” bacteria to most common antibiotics.
The drug is prescribed to children for whom ordinary nasal drops have not helped get rid of a runny nose.
Price
The difference between Isofra antibiotic spray is that it can be combined with other antimicrobial nasal drops. In Russia, prices for healing sprays for professional therapy vary between 400-800 rubles. Isofra nasal spray (15 ml) - the medicine can be ordered online, then the price will be inexpensive. How much does Isofra cost if you buy it in an online pharmacy?

Analogues of the drug Isofra
There is currently no complete analogue of this drug in Russia.
Medicines with similar effects used topically for rhinitis and sinusitis:
- Framinazine. Antibiotic spray made in Belarus. It is based on the same antibiotic framycetin. Available in 10 ml bottles. This analogue is cheaper than Isofra;
- bioparox. It contains the antibiotic fusafungin and is used for diseases of the nose and throat. An effective drug. Among the disadvantages, it is worth noting the high price of the medicine;
- protargol (Sialor). A unique drug with silver ions and a strong antiseptic effect. Its purpose still causes controversy among doctors regarding the harmlessness of the silver contained in it. However, Protargol helped many patients cure severe runny nose and sinusitis;
- polydex. A stronger drug with more restrictions, contraindications and side effects;
- vibrocil. This is not an antiseptic or an antibiotic. An exclusively antiallergic drug with a vasoconstrictor component (phenylephrine). It has numerous contraindications: diabetes mellitus, atrophy of the nasal mucosa, cardiovascular diseases and others.
Analogs
You can choose an analogue of a bactericidal spray based on reviews on the Internet. Antibiotics come in 4 different types and separate groups. Storage conditions and shelf life
The instructions for use of the Isofra spray state that if the storage conditions of the drug are violated, the degree of its effectiveness decreases. The bottle should be stored at a temperature no higher than 25 °C, in a dark place, tightly screwed on with the lid and prevent direct sunlight.
Shelf life if the bottle is closed is 1 year. After opening, the shelf life is one month. If you use the medicine after this period, the therapeutic effect may be absent altogether. In addition, side effects may be present. The appearance of allergic reactions often worries precisely those patients who neglect to follow the instructions and use the drug after the expiration date.

Interaction with other drugs
Before you start using Isofra, you need to check for possible reactions resulting from drug interactions.
This is required in order to prevent the development of side effects or complications from various systems and organs. The combination of Isofra with atropine-type drugs, a group of muscle relaxants, anticholinergics and ganlioblockers is not recommended, since there is a possibility of increased reactions from the muscular and nervous systems in the form of neuromuscular blockades, especially against the background of a severe pre-existing neurological clinic.
In addition, doctors do not recommend using several local remedies at the same time; this may lead to an increased reaction from the body or a lack of clinical effect.
Combination with antimycotic agents of various release forms leads to the formation of a reduced therapeutic effect of the latter.
Isofra solution is available in opaque plastic bottles of 15 ml. The container is equipped with a special tip so that the solution can be sprayed like a spray.
No cases of interaction with other drugs have been recorded.
The drug is often used as one of the components of complex therapy.
Pharmacological action and group
Isofra belongs to the group of antimicrobial and antiparasitic drugs; it is an aminoglycoside antibiotic with the active substance framiticin.
In addition to the active substance framiticin, this spray also contains a number of excipients: water, citric acid, sodium chloride, as well as E 218.
It is worth comparing the drug with similar drugs. At the moment, there are no drugs that have exactly the same antibiotic as the main active ingredient, so it is unlikely to find synonyms for the drug on the market. But analogues in terms of efficiency are easy.
- Polydexa is an analogue of Isofra, but contains hormones and phenylephrine. Thanks to this composition, Polydexa is better suited for children.
- Bioparox is also an analogue; moreover, it is able to withstand a wider group of microbes. But it is not intended for a particularly young age, and with prolonged use it begins to become addictive.
The shelf life of the drug is one and a half years from the moment of its immediate release, as well as one month from the moment of its opening. The drug must be stored in a place where small children will not have access to it; its storage temperature should not exceed 25 degrees Celsius.
Isofra does not have a vasoconstrictor effect, that is, it does not eliminate swelling of the mucous membrane and nasal congestion. To combat these problems, it is necessary to take other drugs, for example, Naphthyzin, Nazol, Nazivin, and so on.
Characteristics
Framycetin sulfate is produced in a single dosage form - nasal spray. The medication solution is in a plastic bottle with a spray dispenser. The auxiliary components of the drug are sodium chloride and citrate, methylparaben, citric acid and purified water.
Action
Framycetin belongs to the antibacterial agents from the group of aminoglycosides. This is a broad-spectrum substance. It can damage the cell membrane and cause ion currents to penetrate into the cytoplasm, which stimulates the death of microbes. The drug is active against various pathogens that cause respiratory tract infections:
- Staphylococci.
- Streptococci.
- Klebsiella.
- Corynebacteria.
- Pseudomonas aeruginosa.
In addition, framycetin has bactericidal properties against E. coli, Shigella, Salmonella, and Yersinia. Thus, the spectrum of antimicrobial action includes both gram-positive and gram-negative flora.
Distribution in the body
The drug has an exclusively local effect. Framycetin practically does not penetrate into the systemic circulation (absorption is extremely low, in trace amounts). Therefore, pharmacokinetic studies have not been conducted.
Reviews about the drug
I never thought that a child’s runny nose could last so long. When my child came home from kindergarten with snot, I somehow didn’t take it seriously. This happened often, and we usually dealt with such nasal discharge by using vasoconstrictor drops. But this time they did not help and, moreover, the snot became thick and green. A little over a week later, we went to the doctor, and he told us that we had an infection in our nose, and prescribed us Isofra, a nasal spray. The child began to breathe easier the very next day, then the green color of the snot disappeared, and after five days there was nothing left of such a severe runny nose. These drops dealt with such a severe runny nose in just 4-5 days.
Elena, Novosibirsk.
I use this drug to treat adenoids in a child. He has been suffering from them for quite some time, but we still can’t get rid of them. Isofra seems to help, and very quickly. After using the medicine, my son immediately begins to breathe easier. But, unfortunately, he does not completely get rid of them. As soon as we stop treatment with this spray, the adenoids return. Last summer I had hope that they had disappeared completely, but as soon as autumn and cold weather arrived, they returned again. As for this particular drug, it is certainly effective, but for some reason it acts as a symptom for us. I expected more from an antibiotic, even if applied locally.
Ekaterina, St. Petersburg.
Everyone knows that children get colds more often than adults. Probably because the child has not yet developed immunity to such diseases. They proceed in different ways. It happens that a cold goes away in a few days, even with minimal treatment, and it also happens that even the discharge of snot from the nose cannot be overcome for a long time.
This is what happened to my child.
We defeated all the main symptoms of the disease, such as cough and fever, but the snot remained. And even moreover, they became thick and dark. Then the doctor prescribed Isofra for us. And in just four days there was nothing left of the nasal discharge. The drug is safe, although it is an antibiotic; we had no side effects or dysbacteriosis. It’s a pity the price is not cheap – 300 rubles and that’s for a small bottle. Olga, St. Petersburg.
Isofra nasal spray, price and analogues
Among the many positive reviews about this drug , which speak about its effectiveness and safety for the child, you can also find those in which the high price of the spray is called a negative point. Indeed, on average in Russia the drug can be purchased at a price of 280 to 320 rubles, which is not so cheap for a 15 ml bottle.
This medicine also has analogues, but not all of them are cheaper, there are drops and sprays in the same price category, and some drugs are even more expensive. Analogs can also be useful when Isofra is ineffective or there are contraindications to its use. And here is what the list of the most famous such drugs looks like.
- Polydexa,
- Rinofluimicil,
- Sialor or Protargol,
- Octenisept,
- Chlorophyllipt oily,
- Okomistin are eye drops, but they are also used to treat the nose,
- Collargol,
- Bioparox,
- Miramistin,
- Bactroban.
Choosing an analogue of funds is not as simple as it seems at first glance. After all, different medications used to treat the same disease have different compositions, different contraindications and different age restrictions, so a doctor is best able to choose an analogue. And you just need to explain the reason why the prescribed remedy is not suitable.
Isofra drug
Overdose
When this drug is used correctly (in aerosol form), cases of overdose have not been described.
If a child unintentionally ingests medication, seek immediate medical attention.
It should be remembered that during treatment, resistant strains of microorganisms may appear.
The use of Isofra should only be carried out under strict medical supervision.
Important information:
- Long-term use leads to dysbacteriosis of the nasal cavity.
- Isofra does not affect the ability to drive a car. Only with the development of neurological manifestations or damage to the muscular system against the background of side effects, it is recommended to refrain from driving.
Preventive use of Isofra
Prophylactic use is justified if the patient has undergone surgery in the nasopharynx area. Often after this type of operation, inflammatory processes develop in the nose and pharynx. Therefore, for prevention, otorhinolaryngologists prescribe Isofra.
In reviews, patients report that after removal of the adenoids, the drug is often prescribed to both children and adults. At the same time, they receive treatment with sprays and drops with a disinfecting and moisturizing effect. As a result of this therapy, it is possible to avoid complications and restore the condition of the mucous membrane and healthy breathing as quickly as possible.
