The American Academy of Environmental Medicine officially confirms that the increase in gastrointestinal diseases is associated with the cumulative effect of unnatural food. But supermarkets leave the buyer no choice. As a result, there is a growing need for drugs that restore the functioning of the gastrointestinal tract. According to public reviews, Motilium is in the top five among drugs for vomiting and nausea prescribed for children.
Composition and release form
The drug Motilium has four formulations, but in all of them the active ingredient is domperidone. The drug does not have any other active ingredients, but there are auxiliary ingredients, the set of which depends on the release form of Motilium:
- Tablets to swallow are biconvex, white or beige in color, and the sides of the tablets are engraved with “Janssen” and “M/10.” It also contains lactose, hypromelose, microcrystalline cellulose, magnesium stearate, hydrogenated vegetable oil, sodium lauryl sulfate, polyvidone, corn and potato starch.
- Lozenges are white, matte, round in shape. They additionally contain mannitol, aspartame, gelatin, poloxamer 188 and mint essence.
- Suspension for children (also called syrup for children) is a liquid in a special bottle. In addition to domperidone, it contains sodium hydroxide and saccharinate, methyl parahydroxybenzoate, carboxymethylcellulose and the drug is based on water.
It is better to select the form of Motilium depending on the reason for prescribing the medicine and the age of the person (in a child, tablets can further cause a gag reflex, and in syrup it is convenient to use the drug at any age).
pharmachologic effect
Manufacturer: Catalent/Janssen-Cilag, UK or Italy
Release form: tablets, dispersible tablets, suspension
Active ingredient: domperidone
Synonyms: Motilak, Motonium, Passazhiks, Motizhekt, Motinorm, Motogastrik, etc.
The drug is a dopamine antagonist with antiemetic properties. Contains domperidone, which accelerates intestinal motility, improves the movement of food and prevents stagnation in the stomach.
At the same time, Motilium does not change gastric secretion and does not affect the production of hydrochloric acid, like proton pump inhibitors (Omez, Nolpaza, etc.).
Activity in the body
The drug Motilium acts as an antiemetic and relieves nausea and heaviness in the stomach due to its component composition, or rather domperidone. It helps cleanse the stomach and close the sphincter valve so that food is not thrown back into the esophagus and does not provoke heartburn and other unpleasant sensations.
The maximum concentration of the drug in the body is reached 1-1.5 hours after administration , and is completely eliminated after 7-9 hours. But such a good concentration will occur in a short period of time if you take the medicine correctly - before meals, otherwise the absorption of the drug will be reduced.
Regarding the withdrawal time of Motilium, everything is very individual, if a person has problems with kidney and liver functions.
Regimen and dosage
Motilium Express is dissolved half an hour before meals, and in no case should the medicine be taken after lunch or dinner. The lower the acidity of the stomach, the worse the drug is absorbed.
The tablets are fragile, so it is not recommended to squeeze them out of the cell. It is better to remove the foil from the cell and take out the drug. Motilium is immediately placed under the tongue, where the medicine dissolves in a few seconds. There is no need to drink water.
Instructions for use of the antiemetic Motilium Express contain all the necessary instructions:
- Patients over the age of 12 years when vomiting take 1 tablet 3 times in 24 hours. The appointment lasts 7 days. If Motilium Express does not work, you need to contact a specialist.
- Children from 5 to 12 years old are prescribed the same dosage. In this case, the condition is met: the child’s weight must be above 35 kg.
Against the background of kidney dysfunction, the dose is reduced to 1 tablet 1-2 times a day. The dosage is also reduced in case of severe liver damage.
Indications for use
The use of the drug Motilium should be carried out according to the instructions, which spell out what the drug helps with.

This is what Motilium cures. But you can also drink it to speed up the digestion process. Motilium is also used in complex treatment to eliminate stomach and duodenal ulcers, but here the prescription must be carried out by a doctor.
Analogues of Motilium
The medicine is used to eliminate many dyspeptic symptoms. The cost of 30 tablets is quite high. For an adult they last up to 10 days. Motilium substitutes differ in active substances, pharmacological group, and indications. Depending on the symptoms and cause, the following groups may be prescribed:
- enzyme preparations;
- synonyms – analogues according to INN;
- antidiarrheals;
- antacids;
- antispasmodics;
- sorbents;
- agents that regulate intestinal motility.
The choice of a cheap Motilium substitute should be entrusted to a gastroenterologist to determine the cause of dyspepsia and comprehensive treatment.
Motilium and structural analogues can be used on their own for up to 28 days without a doctor’s prescription, provided there are no side effects or worsening of the condition.

Representatives:
- Pancreatin;
- Creon;
- Mezim;
- Enzistal;
- Festal;
- Hermital;
- Panzinorm et al.
Medicines compensate for the lack of enzymes necessary for the digestion and absorption of food and eliminate dyspeptic symptoms. Disadvantage: with constant use and abuse, the synthesis of enzymes in the body is disrupted.

Motilium or Motilak – which is better?
Manufacturer: Veropharm, Russia
Release form: tablets
Active ingredient: domperidone
Both drugs are used to restore gastrointestinal contractility. Eliminate abdominal pain, heartburn, bloating, heaviness in the abdomen and nausea. There is only one active ingredient – domperidone.
The difference between drugs is in the form of release, manufacturer and pricing policy. Motilium is a regular and orally soluble tablet and suspension. Motilak is available in film-coated tablets. The cost of the second one is lower.
Motilium and Motilak are synonyms. However, for the quality, variety of release forms and the ability to use the express formula in early childhood, many patients and doctors prefer Motilium. The quality and effectiveness have been confirmed by clinical studies, on the basis of which the instructions have been compiled.

Contraindications
Drinking Motilium is strictly prohibited for people with:
- Bleeding in the gastrointestinal tract.
- Prolactinoma.
- Mechanical intestinal obstruction.
- Perforation of the intestines or stomach.

Contraindications may vary from person to person, so you should consult your doctor before taking the drug.
Pharmacokinetics
Motilium is quickly absorbed from the gastrointestinal tract. Maximum accumulation in plasma is observed an hour after a meal. The drug is characterized by fairly low bioavailability - about 15%. If the patient has low acidity, the absorption of domperidone is markedly reduced.
The active substance binds to blood plasma proteins within 91–93%. It is quickly distributed in tissues, but at this time it is weakly concentrated in the brain area. Metabolism of the drug components is carried out by the intestines and liver. Motilium is excreted in feces up to 66%, in urine - about 33%, unchanged. The half-life of the drug is 7–9 hours. In patients diagnosed with severe renal failure, this figure increases even more.
Side effects
Motilium is well tolerated by people, but in some cases it can cause:
- Rash in the form of urticaria.
- Intestinal spasms.
- Amenorrhea.
- Gynecomastia.
- Hyperprolactinemia.
- Extrapyramidal disorders.
Side effects from taking Motilium occur extremely rarely and more often with an overdose of the drug, or if its composition is not suitable for the person or the patient’s body itself suffers from other functional disorders.
Side effects
The medicine "Motilium", instructions and patient reviews indicate this, can cause the following negative effects:
- hyperprolactinemia;
- intestinal spasms;
- allergies;
- amenorrhea,
- passing extrapyramidal disorders in children;
- gynecomastia;
- problems in the functioning of the stomach and intestines;

If the drug is taken excessively, drowsiness, disorientation, and extrapyramidal abnormalities are observed (usually in children). These symptoms are treated by gastric lavage.
Overdose
In case of an overdose of Motilium, a person may experience disorientation, drowsiness, and extrapyramidal reactions. If the first two symptoms of a drug overdose appear, you must rinse your stomach, drink activated charcoal and stop taking the medicine.
If a person has extrapyramidal reactions, then the help of specialists is needed; only they can prescribe the correct treatment to avoid negative consequences for the human body.
Use in children
Instructions for children regarding the use of Motilium suspension for diarrhea state that the drug can be prescribed to children for various manifestations of hypotension of the gastric walls, which are associated with difficulties in the evacuation of products from the stomach into the duodenum.
As a symptomatic medication, such a drug is prescribed to relieve dyspeptic disorders caused by the following phenomena:
- nutritional disorders of nursing mothers;
- motion sickness in transport (Motilium quickly helps with vomiting in a child);
- improper complementary feeding in the first year of a newborn’s life (the instructions for Motilium regarding the use of a suspension for children with diarrhea directly indicate this);
- carrying out medical procedures (endoscopy);
- taking certain medications that provoke cyclic vomiting.
The drug can be used during the neonatal period, but strictly as prescribed by the attending physician and under his supervision.
For infants 2–3 months of age, Motilium is recommended for extensive and frequent regurgitation. The medication must be taken before feeding. It cannot be mixed with milk or complementary foods.
Motilium for vomiting in a 3-year-old child is prescribed only in the form of a suspension. Then its dosage is 0.25–0.5 ml per kilogram of the baby’s weight. The amount of suspension is measured using a special dispenser syringe with dosage divisions marked on it, taking into account the child’s body weight (in increments of 1 kg). The medicine is prescribed before meals (15–30 minutes before it) three times a day.
Sometimes it is possible to increase the amount of the drug, but it is necessary that it does not exceed the daily volume (2.4 ml per kilogram of weight). If the child is over 5 years old, but body weight does not exceed 35 kg, then the dosage is set in a similar amount.
For children of the older age group, the drug is prescribed according to the following scheme:
- in the period of 5–12 years, you should take a tablet before each meal (4 times);
- after the age of twelve, 2 tablets are prescribed, but only three times a day.
The duration of the therapeutic course depends on the severity of the symptoms. The minimum duration of a therapeutic course for children is limited to a term per week.
Motilium is prohibited for use if the child has a pathology associated with dysfunction of prolactin synthesis (hyperprolactinemia). An absolute contraindication is also prolactinoma, a tumor of the anterior pituitary gland, which is often benign in nature.
Parents are obliged to scrupulously monitor compliance with the dosages recommended by the pediatrician, since exceeding them can provoke serious consequences:
- speech disorder;
- convulsive syndrome;
- confusion;
- violation of spatial orientation.
If you unintentionally exceed the prescribed dose, you must immediately organize gastric lavage. For the procedure, you need to use a saline solution, and then give the child any sorbent to drink. After providing emergency care, you should definitely consult with a specialist.

Motilium or Domperidone – which is better?
Manufacturer: Alsi Pharma, Ozon, Russia
Release form: tablets
Active ingredient: domperidone
Domperidone and Motilium - drugs with the same composition, are synonymous. They have the same indications, contraindications, and side effects.
A significant disadvantage of the domestic analogue of Domperidone is the low quality of raw materials (from India or China) and the frequent occurrence of side effects. The cost of the substitute is several times lower.
Another difference between Domperidone and Motilium is the release form. Pharmacies stock only regular tablets; the imported analogue is presented in display cases as dispersible tablets and syrup for children.
Analogue Metoclopramide
This drug is also a cheap analogue containing metoclopramide hydrochloride. The indications for use of this substitute are the same as Motilium, but the contraindications are somewhat different. For example, Metoclopramide should not be taken if you have bronchial asthma or during pregnancy.
The number of possible adverse reactions is slightly greater than that of the original. For example, when taking it, people may experience extrapyramidal symptoms and spasms of the facial muscles. The price of the drug is about one hundred and twenty rubles. Thus, the main advantage of this substitute is its price, and the disadvantage is that it has more contraindications than Motilium.
What kind of drug
Motilium is a drug that improves intestinal motor function, has an antiemetic effect, it increases the tone of the sphincter of the lower part of the esophagus. It does not affect the activity of gastric juice and its production.
The active substance of the drug is domperidone (a substance that has antipsychotic properties, but does not have side effects). Taking the drug increases the production of prolactin.
Available in the form of tablets, lozenges and suspension. Each form has different dosages of the active substance and differs in the country of origin.
The most pronounced therapeutic effect is increased peristalsis, which helps accelerate the removal of food from the stomach and duodenum. With rapid evacuation, food does not rot or ferment in the digestive tract, which helps reduce heartburn, nausea and increased gas formation.
Motilium helps improve motility of the gastrointestinal tract, so that food does not have time to ferment and does not cause heartburn, nausea and flatulence
Use during pregnancy
Motilium is not recommended for diarrhea during pregnancy, because There is insufficient data on its effect on the fetus.
Therefore, along with the fact that the medicine has many indications and helps to cope with various painful conditions, its use during pregnancy is allowed when the expected effect outweighs the possible risks.
To date, there is no confirmed data on the effect of medication on a child during breastfeeding. Therefore, if the drug is used in the treatment of diarrhea, feeding should be stopped at this time.
Purpose of the drug
Take for the following conditions and diseases:
- Disorders of the digestive process due to inflammation of the esophagus, movement of stomach contents in the opposite direction or stagnation. Accompanied by bloating and pain.
- Feeling nauseous and vomiting due to medications used to treat Parkinson's.
- The presence of infectious diseases, food poisoning, intoxication, functional disorders of the digestive system. When side effects occur from taking medications, chemotherapy or radiotherapy, it is accompanied by vomiting.
- Problems with the natural movement of food through the gastrointestinal tract, when there is pain in the abdomen with vomiting.
Contraindications and side effects
Motilium has a fairly wide range of contraindications:
- a tumor in the pituitary gland that produces prolactin;
- simultaneous use with Motilium of tablet forms of fluconazole, ketoconazole, voriconazole, erythromycin, amiodarone, clarithromycin, telithromycin;

The medicine is contraindicated in any cases where it is prohibited to stimulate intestinal motility. You should also be especially careful when using Motilium if you have kidney problems, problems with heart rhythm, or heart failure.
According to the results of medical studies, very rare negative effects occur when using Motilium - no more often than 1% of small patients . This:
- headaches, weakness, tearfulness, anxiety, drowsiness, leg cramps;

Mental disorders in children can also be caused by an overdose of the drug, but when contacting a doctor, it is imperative to exclude more serious causes of such neurological symptoms.
They appear extremely rarely (mainly in babies under one year):
- strong excitement with whims, crying, fear;

We are not talking about the patient “dictating” his choice to the doctor, but the pricing policy of the pharmaceutical industry does not yet provide stable information on the cost of drugs. This means that doctors have to adapt and look for cheaper substitutes. Otherwise, people give up treatment due to the inability to purchase an imported drug at a high price.
Compound
The doctor can prescribe to the patient any dosage form of the drug "Motilium", the active substance in the composition predominates one, but in different dosages. The active component of the drug is domperidone.
"Motilium" suspension contains 1 mg of active substance per 1 ml of solution. Each swallowable pill contains a dosage of domperidone equal to 10 mg. Motilium lozenges contain the same portion of the active ingredient.
In addition to the specified substance, Motilium contains other substances in one tablet or serving of suspension: asparkam, sorbitol, polyvidone, magnesium stearate, lactose, etc.
How does domperidone work?
Domperidone blocks dopamine receptors in the upper digestive tract. This causes contraction of the muscles at the entrance to the stomach and relaxation of the muscles at the exit from it, and also enhances the peristalsis of the stomach itself. Therefore, the use of Motilium for diarrhea is inappropriate. These actions speed up the passage of food through the stomach into the intestines, which helps prevent vomiting and reduce nausea, bloating and fullness. Domperidone also prevents the backflow of food from the stomach into the esophagus.
Motilium also blocks dopamine receptors in the area of the brain that are activated by nerve signals from the stomach. Once activated, it sends signals to the vomiting center. Blocking dopamine receptors prevents nausea and vomiting.
How to take it correctly
Tablets and suspension should be taken 15–20 minutes before meals. Food may interfere with the absorption of the active substance, so the drug should not be taken during meals. The suspension must be taken by first measuring the required amount using a measuring syringe. Tablets for oral administration should be taken with clean water at room temperature. It is not recommended to drink juices or milk. Lozenges should be held on the tongue until completely dissolved. They are not recommended to be chewed or swallowed whole.
The author of these lines takes Motilium in tablet form, intended for resorption. This method is the most convenient in practice. The tablet dissolves quickly on the tongue and has a pleasant minty taste. I take Motilium for nausea caused by exacerbation of gastritis. I always take the drug on the road.
Composition and description of the medication
The positive properties of the drug are achieved due to the “rich” composition:
- starch;
- vegetable oil;
- magnesium stearate;
- sodium lauryl sulfate;
- polyvidone;
- hypromelose.
Motilium is available in several forms:
- suspension;
- tablets for oral administration;
- lozenges.
Below in the table we list the components included in other forms of the medication:
Suspension | Lozenges | Tablets for oral administration |
sodium saccharin | mint essence | lactose |
sodium hydroxide | mannitol | cellulose |
sorbidol | gelatin | |
sodium carboxymethylcellulose | aspartame | |
propyl parahydroxybenzoate | polaxamer |
Many people call the suspension syrup or children's Motilium, as it is prescribed by pediatricians for the treatment of children. Lozenges are brown in color and round in shape, with an inscription on the sides.
The coated pills are also round in shape and painted white. On the break there is a tablet of a uniform shade without inclusions.
The light-colored suspension has a consistency similar to jelly. The product is available in a 100 ml volume in a dark glass jar. If the pills are allowed to be taken by children over 12 years of age, then the suspension is also acceptable for babies up to one year old to prevent colic, since it contains less of the active drug.

List of analogues of this drug
The drug has analogues of foreign and domestic production. Motilium analogues (for example, Motilak, Cerucal, Domperidone, Metoclopramide or Ganaton) are very often prescribed as a replacement for this medicine. In our country, Motilak and Domperidone are produced - these analogues are not only domestic, but also cheap compared to the original Motilium. Drugs in the form of “Ganaton” and “Cerucal” are foreign-made drugs. As for Metoclopramide, it is produced both in Russia and abroad. Russian analogues of Motilium are no less effective.
Among other things, if necessary, the medication can be replaced with generics, which also contain domperidone. We are talking about such drugs as Gastropom, Domidon, Domrid and Brullium. In addition, analogues of Motilium are Intanorm, Motizhekt, Perilium, Nausilium, Passazhiks and others.
The drug "Motilium" differs from the above products from the list mainly only in price and manufacturer. The effectiveness of all these drugs has been proven by scientific research. Next, let's look at Motilium analogues in more detail.
Use of Motilium for newborns
For babies, the need for Motilium occurs with increased regurgitation, insufficient weight gain, diarrhea with the threat of dehydration, and intestinal colic.
Pediatricians prescribe a suspension at the rate of 0.25-0.5 ml/kg. It is given before feeding 3-4 times a day. The child is weighed before the appointment.
The syrup packaging contains a measuring syringe, on which divisions are applied: on the one hand, by weight, on the other, by dose. The bottle should be shaken well before putting into the syringe. The drawn-in amount is poured into a spoon or drops directly into the baby’s mouth. No drinking required. The syringe should always be washed with warm water without detergents.
For regurgitation and colic
The dosage is determined by the weight of the baby according to the method described above. Three applications per day are usually sufficient. If the baby is excited and screams at night, it is allowed to give the dose a fourth time. You can try to get by with drops of Espumisan Baby. The drugs are compatible and do not cause additional negative reactions.
To normalize intestinal motility, you may need to take small doses of Motilium for three months.
For heartburn
The feeling of burning behind the sternum is familiar to almost every person. For some people, it occurs constantly after eating. Unpleasant sensations are a consequence of the reflux of (involuntary) stomach contents into the esophagus. The cause of this condition can be obesity, stress, alcohol abuse, and poor nutrition. Regardless of why the burning sensation occurs, Motilium will help with heartburn. To completely get rid of it, taking medication is not enough. You need to normalize your diet and give up bad habits.

special instructions
It is not advisable to combine the drugs with the following medications:
- broad spectrum antibiotics,
- antiarrhythmic,
- antidepressants,
- antifungal,
- antimalarial,
- antiallergic,
- antitumor.
Motilium should not be taken to prevent nausea after anesthesia. The active substance can provoke the appearance of tachycardia with long-term use; this should be taken into account by persons with a predisposition to heart disease. If arrhythmia occurs, treatment with the drug must be stopped.
It is not recommended to exceed the minimum dose of the medication to avoid side effects.
The main component affects the speed of reactions and concentration, so during treatment you should refrain from driving a car or hazardous activities.
Taking antibiotics simultaneously with Motilium is not recommended
Interaction with other drugs
Some antitussives may significantly reduce the effectiveness of Motilium. Azole antifungals (fluconazole, ketoconazole, etc.), macrolide antibiotics (erythromycin, etc.), HIV protease inhibitors, calcium antagonists, as well as amiodarone, aprepitant, nefazodone, telithromycin can excessively increase the amount of domperidone in the plasma. In addition, some of the medications listed can disrupt the heart rhythm.
Motilium can be combined without fear with antipsychotics, bromocriptine and levodopa.
Prices for the drug and its main analogues, average in Russia
One of the reasons for replacing the drug is its high price. Information on prices for Motilium and its analogues is given in Table 1.

As can be seen from the table, Motilium is the most expensive drug compared to its analogues. There are cheaper antiemetics. Some of them contain the same active ingredient – domperidone. There are also drugs that contain other active ingredients (prinfia bromide, bromopride), but their effect is similar to that of Motilium.
There are also analogues that are not sold in the Russian Federation (Domrid, Nausilium, Bromoprid, Brulium lingvatabs and others). If necessary, such a drug can be ordered online or purchased in another country.
